SNF Physician, Tampa Bay
Overview
Team Florida is looking for a Skilled Nursing Facility Physician to lead care for our senior patients as they move through post acute stays across the Tampa Bay market. This is a value based role built around fewer patients per physician, longer time at the bedside, and a full interdisciplinary team supporting every resident, so success is measured by healing, stability, and safe transitions home rather than volume. You will round at a focused set of partner facilities across Tampa Bay, working side by side with facility nursing staff, therapists, and our own care coordination team to keep patients healthy and out of the hospital. This is a pivotal seat as Team Florida builds a stronger, more connected care model for seniors throughout the Tampa Bay community.
What you will own
- Round on an assigned panel of skilled nursing facility patients across Tampa Bay partner buildings, providing daily or as needed medical oversight
- Conduct thorough physical exams, medication reconciliation, and care plan updates for each resident under your care
- Lead admission evaluations for patients transitioning from hospital to skilled nursing care, setting the tone for a proactive recovery plan
- Partner with facility nursing staff, therapists, social workers, and dietitians to build individualized, whole person care plans
- Communicate directly with hospitalists, primary care physicians, and specialists to ensure continuity as patients move between care settings
- Identify early signs of clinical decline and intervene proactively to prevent avoidable hospital readmissions
- Document thoroughly and promptly to keep the entire care team aligned on each patient's status and goals
- Support family and patient conversations around goals of care, discharge readiness, and long term planning
- Participate in quality and outcomes reviews with Team Florida's clinical leadership, sharing one scorecard focused on patient wellbeing
What we look for
- Active and unrestricted Florida medical license
- MD or DO degree from an accredited medical school
- Board certification or board eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine
- Active DEA registration
- Experience caring for geriatric or medically complex patients in a skilled nursing or post acute setting preferred
- Comfort working within a value based, team based care model focused on outcomes rather than visit volume
- Strong communication skills for coordinating across facility staff, specialists, and family members
- Willingness to travel between partner facilities within the Tampa Bay market
- Current BLS certification
