Primary Care Physician, West Sanford

Overview

Team Florida is seeking a full time Primary Care Physician to lead senior focused care at our West Sanford clinic, part of the Orlando market. This role operates inside a true value based model, with a panel sized for depth rather than volume, longer visits, and a full care team built around every patient. You will practice in West Sanford, a community of longtime Seminole County families and a growing senior population who deserve a physician who knows them well. This is a seat for a physician ready to build lasting relationships with patients, lead a care team toward shared quality goals, and help shape a new chapter for primary care in this market.

What you will own

• Manage a dedicated patient panel sized to allow comprehensive, relationship based primary care rather than high volume visits
• Conduct longer, unhurried office visits that make room for chronic disease management, preventive screening, and behavior change conversations
• Partner with a care team of nurses, medical assistants, care coordinators, and pharmacy support assigned to your panel
• Develop and own individualized care plans for patients with complex and chronic conditions common among seniors
• Review quality and outcomes data regularly with clinic leadership, using one shared scorecard to guide improvement
• Coordinate care across specialists, hospitals, and skilled nursing settings to keep patients healthy and out of the hospital
• Supervise and collaborate with advanced practice clinicians working alongside you in the clinic
• Lead huddles and care team check ins that keep every team member focused on the same patient outcomes
• Engage patients and families in proactive outreach so gaps in care are closed before they become emergencies

What we look for

• Doctor of Medicine or Doctor of Osteopathic Medicine degree
• Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active and unrestricted Florida medical license or active application in process
• Current DEA registration or ability to obtain one prior to start
• Experience or strong interest in value based, senior focused primary care
• Comfort practicing alongside and supporting advanced practice clinicians
• Strong communication skills with patients, families, and a multidisciplinary care team
• Availability for a full time, in clinic schedule in West Sanford
• Bilingual ability in Spanish is a plus given the local patient community
