Team Florida is looking for a Primary Care Physician to lead senior focused care at our Orlando West Sanford clinic. This role sits inside a true value based model, where you carry a smaller panel, spend more time with each patient, and lean on a full clinical team so your energy goes toward outcomes instead of volume. You and clinic operations share one goal and one scorecard, keeping every senior in Sanford healthier at home and out of the hospital. What you will own

  • Serve as the primary physician for a defined panel of senior patients at the Orlando West Sanford clinic
  • Lead longer, unhurried visits that allow real time for listening, examination, and care planning
  • Direct a coordinated care team including nurses, medical assistants, care coordinators, and behavioral health support around each patient
  • Build and manage individualized care plans that emphasize prevention, chronic disease control, and closing gaps in care
  • Review panel level quality and outcome data regularly and adjust care plans to keep patients healthy and out of the hospital
  • Coordinate closely with specialists, hospitalists, and post acute providers to keep care connected across every setting
  • Supervise and collaborate with advanced practice clinicians who extend the reach of the care team
  • Engage patients and families in shared decision making that respects their goals and supports long term wellness
  • Participate in clinic huddles and quality reviews where physicians and operators track the same outcomes together

What we look for

  • Active and unrestricted Florida medical license as an MD or DO
  • Board certification or board eligibility in Family Medicine or Internal Medicine
  • Active DEA registration or the ability to obtain one before starting
  • Experience caring for senior patients, ideally within a value based or Medicare Advantage setting
  • Comfort managing a defined patient panel with a focus on outcomes rather than visit volume
  • Strong skills leading and collaborating with a multidisciplinary care team
  • Experience or genuine interest in working with advanced practice clinicians in a team based model
  • Commitment to full time practice with a consistent daily schedule at the Sanford clinic
  • Strong communication skills for engaging patients, families, and caregivers in their care
