Primary Care Physician, Hyde Park

Overview

Team Florida is looking for a Primary Care Physician to lead senior focused care at our Hyde Park clinic in the heart of Tampa. This role lives inside a true value based model, where a capped patient panel and longer visit times let you practice medicine the way you always wanted to, with a full care team built around you and one shared scorecard for quality and outcomes rather than volume. You will build lasting relationships with the seniors of Hyde Park, South Tampa, and the surrounding neighborhoods, guiding their care proactively rather than reactively. This is a pivotal seat as Team Florida stands up a new company around an already established Tampa practice, giving you the chance to help shape senior primary care across one of Florida's fastest growing markets.

What you will own

• Manage a capped, senior focused patient panel with visit times built for real relationship building and thorough clinical decision making
• Develop and lead individualized, proactive care plans that emphasize prevention, chronic condition management, and early intervention
• Partner daily with a dedicated care team including nurses, medical assistants, care coordinators, and behavioral health support to close gaps in care
• Review quality and outcomes data regularly to guide panel management and identify patients who need extra outreach
• Coordinate closely with specialists, hospitals, and skilled nursing partners to keep transitions of care smooth and safe
• Engage patients and families in shared decision making, health education, and long term wellness goals
• Participate in daily huddles and clinical team meetings to align on patient needs and clinic performance
• Document thoroughly and accurately in the electronic health record to support continuity of care and quality reporting
• Contribute to a culture of clinical excellence by mentoring team members and championing the clinic's care model

What we look for

• Active and unrestricted Florida medical license as an MD or DO
• Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active DEA registration or eligibility to obtain one in Florida
• Experience or strong interest in caring for senior and Medicare eligible patients
• Comfort practicing within a value based, capitated care model focused on outcomes rather than visit volume
• Strong collaborative skills for working within a multidisciplinary care team
• Excellent communication skills for engaging patients, families, and caregivers in their care
• Willingness to use data and technology tools to guide proactive, panel based care
• Availability for a full time, outpatient only schedule in Hyde Park
