Primary Care Physician, Hyde Park

Overview

Team Florida is seeking a full time Primary Care Physician to lead a senior focused panel at our Hyde Park clinic in Tampa. This is a value based seat built around smaller panels, longer visits, and a full interdisciplinary team, where your success and the clinic's success are measured by the same scorecard of patient outcomes and quality. You will practice in one of Tampa's most established and walkable neighborhoods, close to Bayshore Boulevard and downtown, with the resources of a statewide senior care organization behind you. As this practice stands up as a new company built on decades of Florida primary care experience, this is the moment to help shape what senior care in Hyde Park looks like for years to come.

What you will own

• Serve as the lead physician for an empaneled group of senior patients, building long term relationships through regular wellness visits
• Conduct comprehensive annual wellness exams and ongoing chronic condition management with visit lengths built for depth, not volume
• Develop and own individualized care plans in partnership with nurses, care coordinators, behavioral health, pharmacy, and social work on your care team
• Monitor and improve clinical quality measures, preventive screening completion, and gaps in care across your panel
• Coordinate specialist referrals and transitions of care to reduce avoidable hospitalizations and emergency visits
• Document encounters accurately and promptly to support continuity of care and quality reporting
• Participate in regular care team huddles and case reviews to align on high risk and complex patients
• Mentor advanced practice clinicians and support staff within the clinic's team based care model
• Partner with clinic leadership on panel performance, outcomes, and continuous improvement of the care model

What we look for

• Doctor of Medicine or Doctor of Osteopathic Medicine from an accredited medical school
• Active and unrestricted Florida medical license, or ability to obtain one prior to start
• Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active DEA registration or ability to obtain one prior to start
• Experience caring for senior or Medicare Advantage patients preferred
• Comfort working in a value based, team based primary care environment focused on outcomes rather than visit volume
• Strong communication skills and a collaborative approach to working alongside nurses, care coordinators, and specialists
• Willingness to practice full time, Monday through Friday, onsite at the Hyde Park clinic in Tampa
• Commitment to proactive, preventive, relationship based senior care
