Overview

Team Florida is looking for a Primary Care Physician to lead senior focused care at one of our Tampa Bay area clinics. This is a value based seat, built around a smaller patient panel, longer visits, and a full care team that shares one goal and one scorecard with you: keeping every senior in your panel healthy. You will practice relationship centered medicine in a growing Tampa market, with the time and support to know your patients well and manage their whole health picture. This is a chance to help shape a new statewide practice from the ground up while caring for the same community you already know.

What you will own

  • Serve as the primary physician for a manageable panel of senior patients, with visit lengths built for real conversation and thorough exams
  • Lead a dedicated care team of medical assistants, care coordinators, and support staff around your patients
  • Develop and manage individualized, proactive care plans that emphasize prevention, chronic disease control, and early intervention
  • Review patient data and outcomes regularly to guide clinical decisions and close gaps in care
  • Coordinate with specialists, behavioral health, and community resources to address the full picture of each patient's health
  • Educate patients and families on disease management, medications, and healthy living to support long term wellness
  • Participate in care team huddles and clinic quality initiatives aimed at improving patient outcomes
  • Document care thoroughly and accurately using the clinic's electronic health record system
  • Partner with clinic leadership to support the practice's growth and consistent, high quality care across the Tampa Bay market

What we look for

  • Active and unrestricted Florida medical license as an MD or DO
  • Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
  • Active DEA registration
  • Current BLS or CPR certification
  • Experience caring for senior patients, with a genuine interest in geriatric and preventive medicine
  • Comfort with or willingness to grow into a value based care model, including familiarity with Medicare Advantage, HEDIS, and risk adjustment concepts
  • Strong communication skills and a collaborative approach to working within an interdisciplinary care team
  • Availability for full time, Monday through Friday outpatient clinic hours
  • Commitment to quality outcomes and whole person care as the measure of success
