Primary Care Physician, Tampa
Overview
Team Florida is looking for a Primary Care Physician to lead a senior focused panel at our Tampa clinic, part of the Tampa North market. This is a value based seat, which means a smaller patient panel, longer visits, and a full care team built around you so you can focus on keeping patients healthy rather than moving through a schedule. You will practice in a growing statewide network that shares one goal and one scorecard between physicians and operators, with quality and outcomes as the true measure of success. Tampa offers a diverse, fast growing community with a strong senior population, and this is a chance to help shape a new chapter of senior primary care in a market you already know.
What you will own
- Manage a dedicated panel of senior patients with visit times built for thorough, relationship based care rather than volume
- Develop and own individualized care plans that emphasize prevention, chronic disease management, and early intervention
- Partner daily with a care team that includes nurses, medical assistants, care coordinators, and behavioral health support to close gaps in care
- Review quality and outcomes data regularly and adjust care plans to keep your panel healthy and engaged
- Coordinate with specialists, hospitals, and skilled nursing facilities to ensure smooth transitions of care for your patients
- Engage patients and families in shared decision making and proactive wellness planning during and between visits
- Document care thoroughly and accurately in the electronic health record to support continuity and quality reporting
- Participate in practice level huddles and case reviews focused on the patients who need the most attention
- Mentor and collaborate with advanced practice clinicians and support staff working alongside you in clinic
What we look for
- Active and unrestricted Florida medical license as an MD or DO
- Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
- Active DEA registration in good standing
- Experience or strong interest in caring for adults age 65 and older
- Comfort practicing within a value based, panel based care model rather than a fee for service structure
- Strong communication skills and a collaborative approach to working with a multidisciplinary care team
- Willingness to use data and quality metrics to guide day to day clinical decisions
- Current BLS or CPR certification
- Availability for a full time, primarily outpatient schedule in the Tampa North market
