Primary Care Physician, Pinellas Park

Overview

Team Florida is seeking a full time Primary Care Physician to lead a dedicated senior panel at our Pinellas Park clinic, part of the Tampa North market. This is a value based role built around smaller patient panels, longer visits, and a full care team at your side, so your scorecard is patient health and outcomes rather than volume. You will practice in a community of retirees and long time Pinellas County residents who are looking for a physician who knows them by name and has the time to prove it. As this practice grows its presence across Tampa North, this seat offers the chance to help shape senior care for the neighborhoods you already know.

What you will own

• Manage a dedicated panel of senior patients with appointment times built for real conversation and thorough exams
• Lead a care team of nurses, medical assistants, and care coordinators who handle outreach, scheduling, and follow up so you can stay focused on patients
• Build individualized care plans that emphasize prevention, chronic disease management, and early intervention
• Review population health data regularly with your care team to close gaps in care and keep patients on track with screenings and wellness visits
• Coordinate closely with specialists, hospitalists, and skilled nursing teams to keep transitions of care smooth for your patients
• Conduct annual wellness visits and comprehensive assessments that look beyond the exam room at each patient's full health picture
• Partner with clinic leadership on quality and outcomes goals, sharing one scorecard with your care team and the broader organization
• Engage patients and families in shared decision making to support long term health and independence
• Mentor and support advanced practice clinicians and support staff within your care team

What we look for

• Active, unrestricted Florida medical license as an MD or DO
• Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
• Current DEA registration or willingness to obtain one before start
• Experience caring for senior patients, ideally within a value based or managed care setting
• Comfort using data and quality metrics to guide proactive, preventive care decisions
• Strong communication skills and a collaborative approach to working within a multidisciplinary care team
• Availability for full time clinic hours with a commitment to continuity of care for an assigned patient panel
• Willingness to participate in ongoing quality improvement and population health initiatives
• Compassion for the unique needs of older adults, including chronic condition management and social determinants of health
