Overview

Team Florida is looking for a Primary Care Physician to lead senior focused care at our Cypress Creek clinic in the Tampa market. This is a value based seat, meaning you are paid to keep your patients well rather than to move through a full waiting room, so your panel stays purposefully small and your visits run long enough to actually get to the bottom of things. You will work inside a full care team built around you, from care coordinators to pharmacists to behavioral health support, all pointed at the same scorecard of patient outcomes. This is a foundational hire for a new company built on an established Tampa practice, so your voice will shape how senior care is delivered across Cypress Creek for years to come.

What you will own

  • Serve as the primary physician for a capped senior patient panel, building long term relationships rather than one off visits
  • Conduct extended office visits that give you real time to review medications, address chronic conditions, and plan ahead rather than just react
  • Lead a dedicated care team including medical assistants, care coordinators, and behavioral health staff to manage each patient's full health picture
  • Develop and adjust individualized care plans focused on prevention, chronic disease control, and closing gaps in care before they become emergencies
  • Review quality and outcomes data regularly and use it to guide daily decisions for your panel
  • Coordinate closely with specialists, hospitalists, and skilled nursing teams to keep care connected when patients move between settings
  • Participate in regular care team huddles and case reviews to keep everyone aligned on the same goals for shared patients
  • Document care thoroughly and accurately in the electronic health record to support both clinical continuity and quality reporting
  • Mentor and collaborate with advanced practice clinicians and support staff within the clinic

What we look for

  • Doctor of Medicine or Doctor of Osteopathic Medicine degree from an accredited program
  • Active and unrestricted Florida medical license
  • Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
  • Current DEA registration in good standing
  • Current Basic Life Support certification
  • Experience caring for senior and Medicare Advantage populations preferred
  • Comfort working within a value based, team based care model with quality and outcomes as the measure of success
  • Strong communication skills for working with patients, families, and a multidisciplinary care team
  • Willingness to relocate to or currently reside in the greater Tampa area
