Primary Care Physician, Tyrone Gardens

Overview

Team Florida is looking for a Primary Care Physician to lead senior focused care at our Tyrone Gardens clinic, part of our long established Tampa area practice. This is a value based role built around smaller patient panels, longer visits, and a full care team that shares one goal and one scorecard with you: keeping every senior patient healthy. You will practice medicine the way it was meant to be practiced, with the time and support to know your patients well and manage their health proactively rather than reactively. As Team Florida stands up a new company around a practice that has already earned the trust of this community, this is a rare seat for a physician who wants to build something lasting in Tyrone Gardens and the greater Tampa market.

What you will own

• Serve as the lead physician for a defined panel of senior patients, building long term relationships that support proactive, whole person care
• Conduct comprehensive primary care visits with the time needed to address chronic conditions, preventive screenings, and emerging concerns in a single encounter
• Develop and adjust individualized care plans that keep patients healthy and out of the hospital, in partnership with the care team
• Partner daily with advanced practice clinicians, medical assistants, care coordinators, and behavioral health resources embedded in the clinic
• Review quality and outcomes data regularly with clinic leadership to identify patients who need outreach or a change in care plan
• Coordinate closely with specialists, hospitalists, and skilled nursing teams to keep care connected across every setting a patient touches
• Lead daily huddles and care team check ins to align on the needs of high risk and high priority patients
• Document thoroughly and accurately in the electronic health record to support coordinated, value based care
• Mentor and support advanced practice clinicians and clinical staff as a physician leader within the clinic

What we look for

• Doctor of Medicine or Doctor of Osteopathic Medicine from an accredited program
• Active and unrestricted Florida medical license or active application in process
• Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active DEA registration or eligibility to obtain one upon hire
• Experience caring for senior or Medicare eligible patients strongly preferred
• Comfort and enthusiasm for practicing in a value based, team based care model
• Experience with or strong interest in working alongside advanced practice clinicians as part of a shared care team
• Proficiency with electronic health record systems and a commitment to thorough, timely documentation
• Availability for full time practice at the Tyrone Gardens clinic
