Overview

Team Florida is seeking a Primary Care Physician to lead senior focused care at our Tampa North clinic on Broad Street. This is a value based primary care role built around smaller patient panels, longer visits, and a full multidisciplinary team so you can practice medicine the way it was meant to be practiced, with prevention and outcomes as the measure of success rather than volume. You will build lasting relationships with the seniors of Tampa and the surrounding Pinellas communities, guiding their health across every stage of aging. This seat matters now because Team Florida is opening a new chapter for a long established Florida practice, and the physician who joins early will help shape care standards for the market for years to come.

What you will own

  • Manage a dedicated panel of senior patients, with visit lengths and appointment spacing designed for thorough, unhurried care
  • Conduct comprehensive annual wellness visits, chronic disease management, and preventive screenings tailored to each patient
  • Develop and adjust individualized care plans in partnership with a care team of nurses, care coordinators, behavioral health specialists, and pharmacists
  • Monitor and close gaps in care using population health data and quality dashboards shared across the care team
  • Coordinate transitions of care with hospitals, specialists, and skilled nursing facilities to keep patients healthy at home
  • Lead daily huddles and care conferences with clinic staff to align on each patient's needs and goals
  • Document all patient encounters accurately and promptly within the electronic medical record
  • Participate in ongoing quality improvement initiatives that support the clinic's shared outcomes scorecard
  • Build trusted, long term relationships with patients and their families as their primary point of contact in the health system

What we look for

  • Active and unrestricted Florida medical license (MD or DO)
  • Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
  • Active DEA registration
  • Completed residency in Family Medicine, Internal Medicine, or a related primary care specialty
  • Experience caring for senior or Medicare eligible patients strongly preferred
  • Comfort practicing in a value based, team based care model focused on outcomes rather than visit volume
  • Strong communication skills with patients, families, and interdisciplinary care team members
  • Proficiency with electronic medical record systems and quality reporting tools
  • Availability for full time clinic hours at the Tampa North location
