Primary Care Physician, South Stuart
Overview
Team Florida is looking for a Primary Care Physician to lead senior focused care at our South Stuart clinic on the Treasure Coast. This is a value based seat built around smaller patient panels, longer visits, and a full care team that shares one goal and one scorecard with you: keeping every patient healthy. You will practice outpatient medicine in a coastal Martin County community known for its slower pace and strong sense of neighborhood, close to the Indian River and the Atlantic. With Team Florida forming its next chapter as a statewide practice, this is a chance to help shape senior primary care in South Stuart from the ground floor.
What you will own
- Serve as the primary care physician and clinical leader for a defined panel of senior patients at the South Stuart clinic
- Build longitudinal relationships with patients through longer, unhurried visits designed for proactive and preventive care
- Develop and adjust individualized care plans that address chronic disease management, prevention, and whole person health
- Partner daily with a full care team, including nurses, medical assistants, care coordinators, and behavioral health support, around each patient
- Review quality and outcome measures regularly and use that data to close gaps in care before they become emergencies
- Coordinate with specialists, hospitalists, and skilled nursing partners to keep transitions of care smooth for every patient
- Lead daily huddles and care team check ins to align on the needs of the highest risk patients on your panel
- Document thoroughly and accurately in the electronic health record to support coding integrity and quality reporting
- Participate in ongoing training on the value based model, panel management tools, and clinical protocols
What we look for
- Active and unrestricted Florida medical license or eligibility to obtain one
- Board certification or board eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine
- Current DEA registration or ability to obtain one prior to start
- Experience caring for senior patients, ideally within a value based or capitated care model
- Comfort managing a smaller, focused patient panel with an emphasis on outcomes rather than visit volume
- Strong communication skills for collaborating with a multidisciplinary care team
- Willingness to engage with quality metrics, care gap closure, and population health data
- Current Basic Life Support certification or willingness to obtain within the first 90 days
- Availability for a full time, primarily Monday through Friday outpatient schedule
