Primary Care Physician, Seffner

Overview

Team Florida is seeking a full time Primary Care Physician to lead a senior focused panel at our Seffner clinic in the greater Tampa market. This role sits inside a true value based model, which means a smaller patient panel, more time in every visit, and a dedicated care team built around you so your focus stays on keeping patients healthy rather than chasing volume. You will practice in a growing suburban Tampa community where longtime residents and a steadily expanding Medicare population are looking for a physician who knows them by name and manages their whole health picture. This is a seat on the ground floor of a new statewide primary care company, giving you the chance to shape how senior care is delivered across Seffner for years to come.

What you will own

• Manage a capped panel of senior patients with visit lengths built for real relationship building rather than rushed encounters
• Develop and own personalized, proactive care plans that address chronic conditions, preventive screenings, and acute needs in one coordinated approach
• Lead daily huddles and ongoing collaboration with a care team that includes advanced practice clinicians, medical assistants, care coordinators, and behavioral health support
• Close care gaps and drive quality measures through consistent follow up, outreach, and attention to each patient's full health history
• Reduce avoidable emergency visits and hospitalizations through responsive outpatient management and tight coordination with specialists and hospital partners
• Document clearly and work inside shared clinical systems so the entire care team has visibility into every patient's status
• Mentor and guide advanced practice clinicians within the clinic while maintaining full accountability for panel outcomes
• Participate in clinic level quality reviews where physicians and operators work from one shared scorecard focused on patient outcomes
• Build lasting trust with patients and families in the Seffner community through consistent, attentive, in person care

What we look for

• Doctor of Medicine or Doctor of Osteopathic Medicine degree
• Active, unrestricted Florida medical license or the ability to obtain one before start
• Board certification or board eligibility in Family Medicine or Internal Medicine
• Active DEA registration or the ability to obtain one prior to employment
• Residency training in a primary care specialty with demonstrated outpatient experience
• Comfort and experience caring for a senior or Medicare Advantage population
• Genuine interest in value based, team based primary care over fee for service or high volume models
• Ability to collaborate closely with advanced practice clinicians and a multidisciplinary care team
• Bilingual English and Spanish fluency is a plus given the local Tampa market
