Primary Care Physician, Seffner

Overview

Team Florida is looking for a Primary Care Physician to lead senior focused care at our Seffner clinic in the Tampa East and South market. This is a value based seat with a deliberately smaller patient panel, longer visit times, and a full care team built around you so you can practice medicine the way you trained to, proactively and preventively rather than reactively. You will manage your own patient panel from a single clinic in Seffner, backed by nurses, medical assistants, care coordinators, and behavioral health support who share your goal of keeping patients healthy and out of the hospital. As Team Florida stands up a new statewide company around this established practice, this is a chance to help shape a senior primary care model built on outcomes, not volume, right here in Seffner.

What you will own

• Manage a dedicated panel of senior patients with visit lengths built for real relationship building and complex care planning
• Lead proactive, preventive primary care including chronic disease management, annual wellness visits, and early intervention to keep patients healthy at home
• Partner daily with an in clinic care team including nurses, medical assistants, and care coordinators to close gaps in care
• Review panel level quality and outcomes data regularly and adjust care plans to improve patient health and reduce avoidable hospitalizations
• Coordinate with specialists, hospitalists, and skilled nursing partners to keep transitions of care smooth for shared patients
• Document thoroughly and accurately to support quality measures, risk adjustment, and the clinical record
• Engage patients and families in shared decision making and long term health goals rather than single visit fixes
• Mentor and collaborate with advanced practice clinicians and support staff within the Seffner clinic
• Participate in practice level quality meetings and contribute ideas that improve the care model for the whole clinic

What we look for

• Active, unrestricted Florida medical license or ability to obtain one prior to start
• MD or DO with residency training in Family Medicine or Internal Medicine
• Board certification or board eligibility in Family Medicine or Internal Medicine
• Current DEA registration or ability to obtain one prior to start
• Experience or strong interest in senior primary care and value based, panel based practice
• Comfort using clinical data and quality metrics to guide day to day patient care decisions
• Strong communication skills for working closely with patients, families, and a multidisciplinary care team
• Availability for full time practice based at the Seffner clinic
• Basic Life Support certification current or obtained within the first 90 days of employment
