Primary Care Physician, Sanford

Overview

Team Florida is building a senior focused primary care practice in Sanford and is looking for a Primary Care Physician to lead a panel of patients in the heart of the Orlando North market. This is value based medicine from the ground up, with a smaller panel, longer visits, and a full care team built around you so your time goes toward the patients who need it most. You will practice in a Sanford clinic designed for continuity, prevention, and real relationships with the seniors you serve, with quality and outcomes, not volume, as the measure of success. As Team Florida stands up this new statewide practice, this is a seat for a physician who wants to help define what senior primary care looks like in Sanford for years to come.

What you will own

• Lead a dedicated care team of advanced practice clinicians, nurses, medical assistants, behavioral health, and care coordinators around a defined patient panel
• Manage a smaller senior patient panel with extended visit times built for real conversations, thorough exams, and proactive planning
• Develop and own personalized care plans that emphasize prevention, chronic disease management, and keeping patients healthy at home
• Review quality and outcomes data regularly with your care team to close gaps in care and improve patient health
• Coordinate specialty referrals, hospital transitions, skilled nursing facility care, home health, and durable medical equipment needs
• Document thorough, accurate clinical records that support both excellent care and the value based model
• Partner with clinic leadership and operators who share your scorecard for patient outcomes and satisfaction
• Mentor and collaborate with advanced practice clinicians who extend care within your panel
• Engage with the Sanford community to build trust and long term relationships with patients and families

What we look for

• Doctor of Medicine or Doctor of Osteopathic Medicine from an accredited program
• Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active, unrestricted Florida medical license or active application in process
• Valid DEA registration or the ability to obtain one prior to start
• Experience caring for senior patients, with Medicare Advantage or value based care experience preferred
• Comfort practicing alongside advanced practice clinicians in a team based model
• Strong communication skills for building trust with patients and families
• Available for a full time, Monday through Friday outpatient schedule
• Bilingual English and Spanish skills a plus for the Sanford patient community
