Primary Care Physician, Sanford

Overview

Team Florida is looking for a Primary Care Physician to lead a senior focused panel at our Sanford clinic in the Orlando North market. This is a value based seat, built around smaller patient panels and longer visits so you can practice medicine the way you trained to, with a dedicated care team handling the coordination, referrals, and outreach that would otherwise pull you away from patients. You will share one scorecard with your clinic operator, both measured on how well your patients stay healthy rather than how many visits fill your day. Sanford is a growing community anchoring the Orlando North region, and this role gives a physician the chance to build long term relationships with the same seniors year after year.

What you will own

• Manage a dedicated panel of senior patients with visit lengths built for real relationship building rather than rushed encounters
• Lead comprehensive primary care including annual wellness visits, chronic disease management, and preventive screenings tailored to each patient
• Partner daily with a full care team of nurses, medical assistants, care coordinators, pharmacists, and behavioral health specialists built around your panel
• Develop and adjust individualized care plans that address medical, social, and behavioral factors affecting each senior's health
• Review quality and outcomes data regularly with clinic leadership to identify patients who need proactive outreach before small issues become emergencies
• Document thoroughly and accurately in the electronic health record to support coordinated care across the full team
• Collaborate with specialists, hospitalists, and skilled nursing facility teams to ensure smooth transitions of care for patients
• Participate in clinic huddles and care team meetings focused on closing gaps in preventive care and chronic condition control
• Serve as a trusted clinical voice for patients and families navigating aging, chronic illness, and long term wellness planning

What we look for

• Active and unrestricted Florida medical license as an MD or DO
• Board certification or board eligibility in Family Medicine or Internal Medicine
• Current DEA registration or ability to obtain one prior to start
• Experience caring for senior patients, geriatric populations, or Medicare Advantage members preferred
• Comfort practicing within a value based, team based care model focused on outcomes rather than visit volume
• Strong communication skills for building trust with senior patients and coordinating with a multidisciplinary care team
• Proficiency with electronic health record systems and a commitment to thorough clinical documentation
• Availability for full time outpatient practice in Sanford with a schedule built around patient continuity
• Genuine interest in preventive, whole person primary care for an aging population
