Primary Care Physician, Port Saint Lucie

Overview

Team Florida is seeking a full time Primary Care Physician to lead a senior focused panel in Port Saint Lucie, one of the fastest growing retirement destinations on Florida's Treasure Coast. This is a value based seat built around smaller patient panels, longer visits, and a full clinical team at your side, so your success is measured by how well your patients stay healthy rather than how many visits you complete in a day. You will practice in a dedicated primary care clinic serving Medicare age adults, supported by care coordinators, behavioral health specialists, and clinical pharmacists who share your goal and your scorecard. With more than a quarter of Port Saint Lucie residents now 65 or older and new retirees arriving every month, this is the right time to build a lasting medical career in a community that is growing around the needs of seniors.

What you will own

• Manage a dedicated panel of senior primary care patients with extended visit times built for relationship based medicine
• Lead the care plan for each patient, coordinating with care coordinators, behavioral health specialists, pharmacists, and specialists under one shared care plan
• Deliver proactive, preventive primary care including wellness visits, chronic condition management, and early intervention to keep patients out of the hospital
• Address social determinants of health such as transportation, nutrition, and housing that affect a senior patient's wellbeing
• Document care thoroughly and partner with quality coders to ensure each patient's health record reflects the full picture of their needs
• Review quality and outcome metrics regularly with clinic leadership to guide panel management and identify patients who need more support
• Participate in the clinic's on call rotation and collaborate with the broader care team on same day and urgent visits
• Mentor and collaborate with advanced practice clinicians and support staff to maintain a consistent standard of care across the clinic
• Engage with the Port Saint Lucie community to build trust and long term relationships with patients and their families

What we look for

• Doctor of Medicine or Doctor of Osteopathic Medicine from an accredited program
• Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active, unrestricted Florida medical license or the ability to obtain one before start
• Current, unrestricted DEA registration
• Experience or strong interest in value based care and senior focused primary care
• Comfort working within a collaborative, team based care model with shared quality goals
• Willingness to complete a routine TB screening as part of onboarding
• Strong communication skills with patients, families, and a multidisciplinary care team
• Availability for full time clinic hours with participation in a shared on call schedule
