Primary Care Physician, Poinciana

Overview

Team Florida is seeking a Primary Care Physician to lead a senior focused panel at our Poinciana clinic, part of the Orlando South market. This is a value based seat, built around smaller patient panels, longer visits, and a full care team that shares one goal and one scorecard with you. You will practice as the clinical leader for a predominantly Medicare Advantage population, with the time and support to treat the whole person rather than the next appointment. Poinciana is a growing senior community southwest of Kissimmee, and this seat lets you build lasting patient relationships while shaping a new chapter for primary care in Central Florida.

What you will own

• Lead a multi disciplinary care team that includes nurses, care coordinators, behavioral health, and referral specialists around a defined senior patient panel
• Conduct longer, unhurried visits that allow full assessment of physical, emotional, and social wellness at each encounter
• Develop and own individualized care plans focused on chronic disease management, prevention, and keeping patients healthy and out of the hospital
• Partner with in house and network specialists, including cardiology, endocrinology, podiatry, and behavioral health, to coordinate whole person care
• Review quality and outcomes data regularly with clinic leadership to guide panel management and close care gaps
• Document thoroughly and promptly in the electronic medical record to support accurate, coordinated care across the team
• Mentor and collaborate with advanced practice clinicians and support staff within the clinic
• Engage patients and families in shared decision making and preventive health education
• Participate in clinic and market level meetings focused on quality, outcomes, and patient experience

What we look for

• Doctor of Medicine or Doctor of Osteopathic Medicine degree from an accredited program
• Board certification or board eligibility in Family Medicine or Internal Medicine
• Active, unrestricted Florida medical license or the ability to obtain one prior to start
• Valid DEA registration or the ability to obtain one prior to start
• Experience or strong interest in caring for senior and Medicare Advantage patients
• Comfort practicing within a value based, team based, quality driven care model
• Ability to complete the Florida AHCA Level II background check required for clinicians with direct patient contact
• Bilingual English and Spanish skills preferred given the Poinciana patient community
• Strong communication skills and a collaborative approach to working with care team members
