Primary Care Physician, Poinciana
Overview
Team Florida is looking for a Primary Care Physician to lead senior focused, value based care at our Poinciana clinic in the Orlando South market. In this seat you carry a smaller panel of Medicare Advantage patients, spend real time with each one, and lean on a full care team so your medical judgment drives every decision. You and the clinic's operators share one scorecard built on keeping patients healthy, not on volume, which means your day is built around prevention, chronic disease control, and the kind of relationship that keeps seniors well between visits. This is a chance to build a lasting practice and a life in the growing Poinciana community while helping stand up a new statewide organization from the ground floor.
What you will own
- Serve as the primary physician for an established panel of senior Medicare Advantage patients at the Poinciana clinic
- Lead a dedicated care team of nurses, medical assistants, care coordinators, and behavioral health support around every patient
- Conduct longer, unhurried visits that allow for full assessment, education, and relationship building with patients and families
- Develop and manage individualized care plans focused on chronic disease control, prevention, and closing gaps in care
- Partner with specialists, hospitalists, and skilled nursing teams to coordinate care across every setting a patient touches
- Review panel level quality and outcomes data regularly and adjust care plans to keep patients healthy and out of the hospital
- Supervise and collaborate with advanced practice clinicians on the care team as needed
- Engage patients and the local Poinciana community through education and outreach that supports long term wellness
- Document care thoroughly and accurately in the electronic health record to support continuity and quality reporting
What we look for
- Active and unrestricted Florida medical license as an MD or DO
- Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
- Current DEA registration for prescribing controlled substances
- Current BLS certification
- Experience or strong interest in caring for senior patients within a value based or Medicare Advantage model
- Comfort managing complex, multiple chronic conditions common in a geriatric population
- Strong communication skills for working closely with patients, families, and a multidisciplinary care team
- Ability to work full time in an outpatient clinic setting in Poinciana with a schedule free of hospital rounds
- Spanish language skills are a plus for serving the local patient community
