Overview

Team Florida is seeking a Primary Care Physician to lead a dedicated care team serving seniors in Westchester, a longstanding Miami Dade community that is home to one of South Florida's largest concentrations of older adults. This role practices inside a true value based model, with a smaller patient panel, longer visits, and a full multidisciplinary team built around the physician so that quality outcomes, not visit counts, define success. As Team Florida stands up a new company around an established statewide practice spanning 93 clinics in 53 Florida communities, this Westchester seat gives a physician the chance to help shape senior primary care for a vibrant, deeply rooted, largely Spanish speaking community. Physicians and clinic operators here share one goal and one scorecard, both measured by the health of the patients they serve.

What you will own

  • Serve as the lead physician for a smaller, focused panel of senior patients, building long term relationships through consistent, unhurried visits.
  • Partner with a full care team including advanced practice clinicians, nurses, medical assistants, pharmacists, behavioral health specialists, and care coordinators to manage each patient's complete health picture.
  • Develop and continually adjust individualized care plans that emphasize prevention, chronic disease management, and early intervention.
  • Lead comprehensive annual wellness visits, physical exams, and health risk assessments that keep seniors ahead of emerging conditions.
  • Review quality and outcomes data regularly with clinic medical leadership to identify opportunities to improve patient health.
  • Coordinate referrals and communicate closely with specialists, hospitals, and skilled nursing partners to keep care connected across every setting.
  • Address social factors such as transportation, nutrition, and isolation that affect a senior's health, working hand in hand with the care team's community resources.
  • Participate in clinic huddles, case reviews, and peer collaboration to continuously refine the care model for the Westchester patient population.
  • Document care thoroughly and compliantly, supporting the clinic's shared goal of measurable, sustained improvement in patient health.

What we look for

  • Active and unrestricted Florida medical license as an MD or DO.
  • Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Active DEA registration for prescribing.
  • Experience or strong interest in senior focused primary care and value based, team based care delivery.
  • Comfort managing a smaller patient panel with longer visit times and a proactive, preventive care philosophy.
  • Strong communication and relationship building skills, with the ability to connect with a diverse, largely Spanish speaking patient community; bilingual English and Spanish preferred.
  • Willingness to collaborate closely with a multidisciplinary care team and clinic leadership around shared quality goals.
  • Availability for a full time, onsite role at the Westchester clinic.
