Primary Care Physician, Tampa
Overview
Team Florida is looking for a Primary Care Physician to lead senior focused care at our Tampa clinic, part of the Tampa North market serving communities across Hillsborough and Pinellas counties. This is a value based practice built around smaller patient panels, longer visits, and a full care team, where your goal and your operator's scorecard are the same, keeping patients healthy. You will build lasting relationships with a dedicated senior panel, backed by care coordinators, medical assistants, and specialists who share your mission. What you will own
- Serve as the primary physician for a dedicated panel of senior patients, building long term relationships built on trust
- Conduct extended office visits that allow time for a full assessment of physical, behavioral, and social needs
- Develop and manage individualized care plans focused on chronic disease management and preventive screening
- Partner with a full care team including care coordinators, medical assistants, behavioral health, and pharmacy to close gaps in care
- Monitor quality and outcomes metrics for your panel, using data to guide proactive outreach and early intervention
- Coordinate care across specialists, hospitals, and skilled nursing facilities to reduce avoidable hospitalizations
- Engage patients and families in shared decision making about their health goals and treatment plans
- Participate in clinic huddles and team based care planning to align on patient priorities
- Mentor and collaborate with advanced practice clinicians and clinical staff within the practice
What we look for
- Active and unrestricted Florida medical license as an MD or DO
- Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
- Active DEA registration
- Experience or strong interest in caring for senior and Medicare Advantage populations
- Comfort working within a value based, team based care model focused on outcomes rather than visit volume
- Strong communication skills and a collaborative approach with patients, families, and care team members
- Willingness to use data and quality metrics such as HEDIS and risk adjustment to guide patient care
- Current BLS or CPR certification
- Availability for a full time schedule based at the Tampa clinic
