Primary Care Physician, Conway

Overview

Team Florida is seeking a full time Primary Care Physician to lead senior focused care at our Conway clinic in the Orlando South market. This role is built entirely around value based primary care, with a smaller patient panel, longer visit times, and a dedicated care team surrounding every physician so you can focus on outcomes instead of volume. You will practice in the Conway neighborhood of Orlando, a close knit community where our patients have come to rely on a familiar face and a consistent team for their health. As Team Florida establishes its next chapter as a standalone statewide practice, this is a rare seat for a physician who wants to shape senior care in Central Florida and share one scorecard with operators who are just as invested in patient outcomes as you are.

What you will own

• Serve as the lead physician for a smaller panel of senior patients, building long term relationships that support proactive, whole person care
• Conduct longer visits that allow time for thorough assessment, patient education, and shared decision making
• Develop and manage individualized care plans focused on chronic condition management, preventive screenings, and wellness
• Partner daily with a full care team including nurses, care coordinators, and behavioral health support to close gaps in care
• Monitor panel level quality and outcome measures, using data to guide proactive outreach and early intervention
• Coordinate referrals and transitions of care with specialists, hospitals, and skilled nursing facilities to keep patients healthy at home
• Document thoroughly and accurately in the electronic health record to support continuity of care across the team
• Participate in regular care team huddles and case reviews to align on patient goals and outcomes
• Mentor and collaborate with advanced practice clinicians and support staff within the clinic

What we look for

• Active and unrestricted Florida medical license as an MD or DO
• Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
• Current DEA registration in good standing
• Completed residency in Family Medicine, Internal Medicine, or a related primary care specialty
• Experience or strong interest in caring for senior patients within a value based or team based care model
• Comfort using electronic health records and data driven quality measures to guide patient care
• Strong communication skills and a collaborative approach to working within a multidisciplinary care team
• Current malpractice insurance or eligibility to obtain coverage
• Availability for a full time Monday through Friday clinic schedule in Conway
