Primary Care Physician, Conway

Overview

Team Florida is seeking a full time Primary Care Physician to lead patient care at our Conway clinic in the Orlando South market. This is a senior focused, value based seat built around smaller panels, longer visits, and a full care team at your side, so your time goes toward the patients in front of you rather than the clock. You will share one scorecard with your clinic operator, measured by patient outcomes and quality rather than volume, and you will help shape the culture of a new company built on an established, trusted Florida practice. Conway offers a close knit Orlando neighborhood feel with easy access to the wider Orlando South community, making this a seat where you can put down roots and build a lasting patient panel.

What you will own

• Serve as the lead physician for a defined senior patient panel, building long term relationships through comprehensive primary care
• Conduct longer, unhurried visits that allow time for preventive screenings, chronic condition management, and wellness planning
• Partner daily with an onsite care team of nurses, medical assistants, pharmacists, and care coordinators to keep each patient's plan on track
• Direct referrals, hospital and skilled nursing facility transitions, durable medical equipment, and home health services for your patients
• Review and finalize documentation with support from dedicated coding and quality staff so your notes reflect the full picture of patient health
• Participate in a shared on call rotation with physician peers across the Orlando South market
• Monitor quality and outcomes data for your panel and work with your care team to close gaps in preventive care
• Mentor advanced practice clinicians and support staff as part of a collaborative, physician led clinic
• Contribute to center growth by welcoming new patients and representing Team Florida in the Conway community

What we look for

• Doctor of Medicine or Doctor of Osteopathic Medicine from an accredited program
• Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active, unrestricted Florida medical license or eligibility to obtain one
• Current, unrestricted DEA registration
• Experience caring for senior patients in a value based or Medicare Advantage setting preferred
• Comfort practicing within a team based care model with shared goals across clinical and operational staff
• Strong communication skills for coordinating with care coordinators, specialists, and facility partners
• Availability for a full time Monday through Friday clinic schedule with shared on call coverage
• Commitment to preventive, relationship driven primary care for an underserved senior population
