Primary Care Physician, Apollo Beach
Overview
Team Florida is looking for a Primary Care Physician to lead senior focused care at our Apollo Beach Blvd clinic in the Tampa East market. This is a value based seat with a smaller patient panel, longer visit times, and a dedicated care team built around you, so your day is centered on prevention and outcomes rather than volume. You will guide a panel of Medicare age patients through proactive wellness planning, chronic disease management, and coordinated specialist care, all measured against the same quality scorecard shared by our clinicians and operators. Apollo Beach is a growing waterfront community south of Tampa, and this role gives a physician the chance to build lasting relationships with patients while shaping a new statewide care model from the ground up.
What you will own
- Manage a capped panel of senior patients with enough time per visit to address medical and social needs fully
- Lead annual wellness visits and proactive care planning to keep patients healthy and out of the hospital
- Direct chronic disease management for conditions common among seniors such as diabetes, hypertension, and heart failure
- Partner daily with a care team of medical assistants, care coordinators, and referral staff who handle documentation and follow up
- Review quality and outcomes data regularly and adjust care plans to close gaps in preventive screenings and chronic care
- Coordinate closely with specialists, hospitalists, and skilled nursing providers to keep transitions of care smooth
- Build trusted, long term relationships with patients and their families across regular visits
- Participate in practice level quality initiatives and share input on clinical protocols and workflows
- Support a collaborative clinic culture where physicians and staff work toward one shared scorecard
What we look for
- Active and unrestricted Florida medical license as an MD or DO
- Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
- Current DEA registration or eligibility to obtain one
- Experience or strong interest in caring for senior patients in a value based or managed care setting
- Comfort using electronic health records and quality data to guide day to day care decisions
- Strong communication skills for working with patients, families, and a multidisciplinary care team
- Commitment to preventive, relationship based primary care over high volume visit models
- Availability for a full time outpatient schedule at the Apollo Beach Blvd clinic
