Primary Care Physician, Park Avenue

Clinical Leadership Primary Care Apopka, FL FTP

Overview

Team Florida is seeking a full time Primary Care Physician to lead senior focused care at our Park Avenue clinic in Orlando. This is a value based role built around smaller patient panels, longer visits, and a full care team that shares one goal with you, keeping every patient healthier year over year. You will practice in a community where new neighbors are discovering Orlando's mix of lakes, culture, and year round warmth, and where your patients are counting on a physician who has time to know them. This seat matters now because Team Florida is building a new statewide company around an established Florida practice, and Park Avenue needs a physician ready to shape what senior primary care looks like here for years to come.

What you will own

  • Manage a dedicated panel of senior patients with visit lengths built for real relationship and thorough care
  • Lead proactive, preventive primary care including wellness visits, chronic condition management, and early intervention for seniors
  • Partner daily with a care team of nurses, medical assistants, behavioral health, and care coordinators built around your panel
  • Review and close quality and care gaps using a shared scorecard focused on patient outcomes rather than visit counts
  • Develop and adjust individualized care plans for patients managing multiple chronic conditions
  • Coordinate with specialists, hospitalists, and skilled nursing teams to keep care connected across every setting
  • Engage patients and families in shared decision making and ongoing health education
  • Participate in clinic huddles and case reviews that keep the whole care team aligned on each patient's goals
  • Support a welcoming, respectful clinic culture for Orlando area seniors and their caregivers

What we look for

  • Active and unrestricted Florida medical license as an MD or DO, or eligibility to obtain one
  • Board certification or board eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine
  • Active DEA registration or eligibility to obtain one
  • Experience or strong interest in senior focused primary care and value based care models
  • Comfort managing a panel of patients with complex chronic conditions within a team based setting
  • Strong communication skills for building trust with senior patients and their families
  • Willingness to collaborate closely with care coordinators, behavioral health, and specialty partners
  • Bilingual ability in Spanish and English is a plus
  • Availability for a full time schedule based at the Park Avenue clinic in Orlando
