Primary Care Physician, Orlando
Overview
Team Florida is looking for a Primary Care Physician to lead a senior focused panel at our clinic on 17th Street in Orlando. This is a value based seat with a smaller panel, longer visit times, and a full care team built around you so that your energy goes toward relationships and outcomes rather than volume. You and your clinic operator share one scorecard built on keeping patients healthy, so every decision in the exam room is backed by the same goal across the building. Orlando is one of Florida's fastest growing senior markets, and this 17th Street clinic is a chance to build a lasting practice and a lasting reputation in a community that is ready for a different kind of primary care.
What you will own
- Manage a dedicated panel of senior patients and serve as their primary point of clinical accountability
- Conduct longer, unhurried office visits that allow time for a full history, physical exam, and shared decision making
- Develop and adjust individualized care plans for chronic conditions such as diabetes, hypertension, COPD, and heart failure
- Deliver proactive preventive care including annual wellness visits, screenings, immunizations, and early risk identification
- Partner daily with a care team of nurses, medical assistants, care coordinators, behavioral health, and pharmacy to close gaps in care
- Review and act on quality and outcomes data for your panel, using it to guide outreach and same day scheduling decisions
- Coordinate care across specialists, hospitals, and skilled nursing partners so patients experience a connected journey
- Document visits accurately and promptly in the electronic health record to support continuity of care and quality reporting
- Participate in clinic huddles and care team meetings to align on patient priorities for the week ahead
What we look for
- Doctor of Medicine or Doctor of Osteopathic Medicine degree from an accredited program
- Active and unrestricted Florida medical license
- Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
- Active DEA registration or eligibility to obtain one before start date
- Experience caring for senior patients, Medicare Advantage populations, or working in a value based care model preferred
- Comfort working within a smaller panel size focused on quality and outcomes rather than visit volume
- Strong collaboration skills for working closely with a multidisciplinary care team
- Proficiency with electronic health record systems
- Bilingual ability in English and Spanish is a plus for this Orlando patient community
