Overview

Team Florida is seeking a full time Primary Care Physician to lead senior focused care at our North Westmonte clinic in Orlando. This is a value based seat, built around smaller patient panels, longer visits, and a full care team so you can practice medicine the way you always wanted to, with quality and outcomes as the measure of success rather than volume. You will share one goal and one scorecard with your care team and with clinic leadership, all focused on keeping your patients healthy. What you will own

  • Manage a smaller panel of senior patients with the time and support to know each one well
  • Lead longer, unhurried visits that allow for real relationship building and thorough assessment
  • Deliver proactive, preventive primary care including wellness visits, chronic disease management, and screenings
  • Partner daily with a full care team, including nurses, medical assistants, and care coordinators, to close gaps in care
  • Develop and own individualized care plans that keep patients healthy and out of the hospital
  • Review quality and outcome metrics regularly with clinic leadership to guide panel management
  • Coordinate with specialists and community resources to support whole person senior health
  • Mentor and collaborate with advanced practice clinicians on shared patients within the clinic
  • Participate in clinic huddles and care team meetings that keep everyone aligned on patient outcomes

What we look for

  • Active and unrestricted Florida medical license as an MD or DO
  • Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
  • Active DEA registration
  • Experience or strong interest in senior primary care and value based, team based practice
  • Comfort managing chronic conditions common to a Medicare population
  • Strong communication skills for building trust with patients and families
  • Willingness to work collaboratively with a multidisciplinary care team
  • Ability to commit to a full time, Monday through Friday clinic schedule in Orlando
  • Bilingual ability in Spanish is a plus for this North Westmonte patient community
