Primary Care Physician, North Fort Myers

Overview

Team Florida is hiring a full time Primary Care Physician to lead senior focused care at our clinic on N. Tamiami Trail in North Fort Myers, in the heart of the Southwest Florida market. In this value based model you carry a smaller panel of Medicare eligible seniors, spend real time with every patient, and lean on a full care team built around you, because keeping patients healthy, not volume, is how this seat is measured. You and the clinic's operators share one goal and one scorecard built on quality and outcomes, so your clinical judgment drives the plan of care rather than the clock. This is a chance to help build a brand new company around an established Southwest Florida practice, in a growing riverside community that still feels like a neighborhood.

What you will own

• Serve as the primary care physician of record for a panel of senior patients, building lasting relationships through longer, unhurried visits
• Lead a care team that includes an advanced practice clinician, medical assistants, and care coordinators to manage each patient's full range of health needs
• Develop and maintain individualized care plans centered on prevention, chronic disease control, and early intervention
• Review panel level quality and outcomes data on a regular basis and adjust care plans to close gaps and keep patients well
• Coordinate with hospitalists, specialists, and skilled nursing teams so transitions of care stay smooth for patients moving between settings
• Document patient encounters thoroughly and maintain accurate clinical records to support coordinated, team based care
• Participate in clinical huddles and quality meetings that keep the clinic aligned around shared goals for patient outcomes
• Engage patients and their families in shared decision making, education, and support for healthy aging
• Mentor and collaborate with fellow clinicians and staff to strengthen a culture of patient centered, value based care

What we look for

• Doctor of Medicine or Doctor of Osteopathic Medicine degree
• Board certification or board eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine
• Active and unrestricted Florida medical license, or eligibility to obtain one
• Active DEA registration in good standing
• Experience caring for senior or Medicare eligible populations preferred
• Comfort working within a value based, team based primary care model
• Strong communication skills with patients, families, and multidisciplinary care teams
• Commitment to proactive, preventive care and shared accountability for patient outcomes
• Ability to work full time at the North Fort Myers clinic location
