Primary Care Physician, Margate
Overview
Team Florida is looking for a Primary Care Physician to lead a senior focused panel at our clinic in Margate, a Broward County community where nearly a third of residents are age 60 or older. This is a value based seat, built around smaller panels, longer visits, and a full care team so you can practice medicine the way you trained to, with quality and outcomes as the shared measure of success for you and clinic leadership. As Team Florida stands up as its own company around an established statewide practice, this is a rare chance to help shape senior primary care in a community close to Fort Lauderdale and the rest of South Florida. You will build lasting relationships with patients you see often enough to truly know, backed by a team that shares your goal of keeping them healthy.
What you will own
- Serve as the primary care physician for a dedicated senior panel, guiding every aspect of each patient's health under a value based model
- Conduct longer, unhurried visits built for proactive and preventive care rather than high volume scheduling
- Partner daily with a full care team of nurses, medical assistants, care coordinators, and behavioral health support to manage each patient holistically
- Build individualized care plans that address chronic disease management, annual wellness visits, and preventive screenings
- Own quality and outcome metrics for your panel and share one scorecard with clinic operators and leadership
- Coordinate closely with specialists, hospitals, and skilled nursing facilities to keep care connected across every setting
- Engage patients and families in shared decision making and address social factors that affect health
- Lead or take part in daily care team huddles to review high risk patients and close gaps in care
- Document care and track quality measures in the electronic health record to support timely, coordinated treatment
What we look for
- Active, unrestricted Florida medical license as an MD or DO, or a Florida license application in process
- Board certification or board eligibility in Family Medicine or Internal Medicine
- Valid DEA registration, or the ability to obtain one before starting
- Experience or strong interest in senior care and value based or managed care models
- Comfort managing a smaller panel with more time per patient, with outcomes valued over visit count
- Proven ability to work closely within a multidisciplinary care team
- Strong communication skills and a genuine interest in building long term relationships with senior patients
- Bilingual ability in English and Spanish or Haitian Creole is a plus given the Margate community
- Availability for a full time, outpatient clinic based schedule
