Primary Care Physician, Lake Mary

Overview

Team Florida is seeking a Primary Care Physician to lead senior focused care at our Lake Mary clinic, part of a statewide value based practice built around keeping patients healthy rather than counting visits. You will carry a smaller panel, spend meaningfully more time with each patient, and direct a full care team of nurses, care coordinators, and behavioral health support who share your scorecard for quality and outcomes. Lake Mary offers a fast growing Seminole County community with excellent schools, lakefront neighborhoods, and quick access to Orlando, making it a strong place to build both a practice and a life. This is a pivotal seat as our new organization stands up its physician leadership across Central Florida.

What you will own

• Serve as the primary physician for a panel of senior patients, building long term relationships through regular wellness visits and proactive outreach
• Lead a dedicated care team including nurses, medical assistants, and care coordinators to close gaps in care before they become complications
• Spend extended time per visit to manage chronic conditions such as diabetes, hypertension, and heart disease with a whole person approach
• Partner with behavioral health, pharmacy, and specialty resources embedded in the care model to address the full range of patient needs
• Review quality and outcomes data regularly with clinic leadership to guide panel management and identify patients who need more support
• Supervise and collaborate with advanced practice clinicians working alongside you in the clinic
• Participate in daily huddles and care team meetings to coordinate same day access and follow up for your patients
• Engage patients and families in shared decision making around preventive screenings, medication management, and care plans
• Support new patient onboarding and help grow a thriving panel as the Lake Mary clinic expands its presence in the community

What we look for

• Active and unrestricted Florida medical license as an MD or DO
• Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active DEA registration
• Experience caring for senior patients, with a preference for candidates who have worked in a value based or managed care environment
• Comfort leading and collaborating with a multidisciplinary care team including advanced practice clinicians
• Strong communication skills with a genuine commitment to preventive, relationship based primary care
• Willingness to use quality and outcomes data to guide day to day clinical decisions
• Availability for a full time outpatient schedule based at the Lake Mary clinic
• BLS certification current or obtainable prior to start
