Primary Care Physician, Lake Mary

Team Florida is seeking a full time Primary Care Physician to lead senior focused care at our Lake Mary clinic in the Orlando market. This role lives inside a true value based model, where you carry a smaller patient panel, spend real time with every senior in your care, and lean on a dedicated care team so your clinical judgment drives outcomes rather than a clock. As Team Florida establishes this new company around a well known statewide practice, this is a seat for a physician who wants to help shape senior primary care in a market with real momentum.

What you will own

• Manage a dedicated panel of senior patients, building long term relationships that support proactive, whole person care
• Conduct comprehensive visits with extended time per patient, allowing for thorough evaluation, education, and shared decision making
• Develop and adjust individualized care plans that address chronic disease management, preventive screenings, and overall wellness
• Partner daily with a full care team, including nurses, medical assistants, and care coordinators, to close gaps in care and support each patient between visits
• Review quality and outcomes data regularly, using it to guide clinical decisions and improve panel level health
• Coordinate with specialists, hospitalists, and skilled nursing partners to ensure seamless transitions of care for your patients
• Document care thoroughly and accurately within the electronic health record to support continuity and quality reporting
• Participate in clinic huddles and case reviews, sharing insight with fellow physicians and advanced practice clinicians across the Orlando market
• Champion a culture of preventive, relationship driven medicine that keeps seniors healthy and out of the hospital

What we look for

• Active and unrestricted Florida medical license as an MD or DO
• Board certification or board eligibility in Family Medicine or Internal Medicine
• Current DEA registration or eligibility to obtain one prior to start
• Experience or strong interest in senior focused, value based primary care
• Comfort managing a panel of patients with complex, chronic conditions common among Medicare eligible seniors
• Excellent communication skills with patients, families, and a multidisciplinary care team
• Willingness to engage with quality metrics and outcomes based performance as shared measures of success
• Bilingual English and Spanish skills are a plus given the diversity of the Lake Mary and greater Orlando patient population
• Full time availability with a standard Monday through Friday clinic schedule
