Primary Care Physician, Kissimmee
Overview
Team Florida is looking for a Primary Care Physician to lead a senior focused panel at our Oak Commons clinic in Kissimmee, part of the Orlando South market. This is a value based seat, built around a smaller patient panel, longer visits, and a full care team so you can practice medicine the way it was meant to be practiced, proactively and with real time to know each patient. You will share one goal and one scorecard with clinic leadership, and your success is measured by how well your patients stay healthy, not by how many patients you see in a day. Kissimmee is a growing, diverse Central Florida community, and this is a rare chance to build a lasting senior care practice in a market with strong long term demand.
What you will own
- Manage a dedicated panel of senior patients under a value based care model that rewards keeping them healthy
- Lead longer, unhurried visits that allow time for thorough assessment, education, and relationship building
- Direct a full care team including nurses, medical assistants, and care coordinators who support your daily practice
- Build and follow individualized care plans focused on prevention, chronic disease management, and closing care gaps
- Review quality and outcomes data regularly to guide clinical decisions and panel management
- Coordinate with specialists, behavioral health, and community resources to address the whole patient
- Document visits accurately and promptly in the electronic health record
- Participate in care team huddles and case reviews to align on shared patients and shared goals
- Engage patients and families in shared decision making about their health
What we look for
- Doctor of Medicine or Doctor of Osteopathic Medicine degree from an accredited program
- Completion of an accredited residency in Family Medicine or Internal Medicine
- Board certification or board eligibility in Family Medicine or Internal Medicine
- Active and unrestricted Florida medical license or the ability to obtain one
- Current DEA registration and professional malpractice coverage
- Experience or strong interest in caring for senior patients and Medicare Advantage populations
- Comfort practicing within a value based, team based care model
- Bilingual ability in English and Spanish is a plus given the local patient community
- Available for full time practice at the Kissimmee Oak Commons clinic
