Primary Care Physician, Kissimmee

Overview

Team Florida is hiring a full time Primary Care Physician to lead senior focused care at our Oak Commons clinic in Kissimmee, part of the Orlando South market. This is a value based seat built around a smaller patient panel, longer visits, and a full care team at your side, so your time goes to the patient in front of you instead of the clock. You will practice in a growing, newly formed statewide organization where physicians and operators share one goal and one scorecard, keeping patients healthy rather than chasing volume. Kissimmee is a fast growing, family friendly community just south of Orlando with a large senior population that deserves exactly this kind of unhurried, proactive care, and this is a chance to help shape that model from the ground floor.

What you will own

• Manage a dedicated panel of senior patients with visit lengths built for real conversation and thorough exams
• Lead annual wellness visits, chronic condition management, and preventive screenings as the core of the daily schedule
• Partner daily with a care team of nurses, medical assistants, behavioral health specialists, and care coordinators to close gaps in care
• Develop and adjust individualized care plans that reduce avoidable emergency visits and hospitalizations
• Review quality and outcome metrics regularly with clinic leadership to guide panel level health improvement
• Document visits accurately and promptly in the electronic health record to keep the care team aligned
• Coordinate transitions of care with specialists, hospitals, and skilled nursing facilities to protect continuity
• Mentor advanced practice clinicians and support staff within the clinic care model
• Engage patients and families on social and lifestyle factors that affect health alongside clinical treatment

What we look for

• Active and unrestricted Florida medical license
• MD or DO degree from an accredited medical school
• Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active DEA registration
• Experience caring for senior or Medicare Advantage patients preferred
• Background in or strong interest in value based or team based primary care
• Comfort with electronic health record documentation and care team collaboration
• Strong communication skills with patients, families, and interdisciplinary team members
• Availability for a full time weekday clinic schedule in Kissimmee
