Join Team Florida / Physician
Primary Care Physician, Hunters Creek
Overview
Team Florida is seeking a full time Primary Care Physician to lead senior focused care at our clinic in Hunters Creek, a growing community in the Orlando market. This is a value based seat built around smaller patient panels, longer visit times, and a full care team working alongside you so your focus stays on the patient in front of you. You will practice in a model where physicians and operators share one goal and one scorecard, with success measured by patient outcomes rather than volume. As Team Florida establishes itself as a new, locally rooted company, this is a rare chance to help shape the physician experience and clinical culture of a statewide senior primary care practice from the ground up.
What you will own
- Manage a panel of senior patients under a value based model that rewards keeping patients healthy rather than counting encounters
- Conduct longer, unhurried visits that allow time for a full assessment of medical, functional, and social needs
- Develop and oversee proactive, individualized care plans for chronic disease management, preventive screening, and wellness
- Partner daily with a care team of nurses, care coordinators, behavioral health, pharmacy, and social work to close gaps in care
- Coordinate care across hospital, specialty, and skilled nursing settings so patients experience a connected journey
- Review quality and outcomes data regularly with clinical leadership to guide panel level improvement
- Build trusted, long term relationships with patients and families in the Hunters Creek community
- Supervise and collaborate with advanced practice clinicians as part of the care team structure
- Participate in practice level initiatives that advance patient experience, access, and clinical excellence
What we look for
- Active, unrestricted Florida medical license as an MD or DO
- Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
- Active and unrestricted DEA registration
- Experience or strong interest in caring for Medicare age and senior patient populations
- Comfort practicing within a value based, team based care model focused on quality and outcomes
- Strong communication skills with patients, families, and an interdisciplinary care team
- Willingness to use electronic health records and quality dashboards to guide daily care decisions
- Ability to build rapport with a diverse senior patient community in the Hunters Creek and greater Orlando area
- Full time availability with a schedule built around sustainable, no rush patient visits
