Primary Care Physician, Haines City
Overview
Team Florida is seeking a Primary Care Physician to lead senior focused, value based care at our Haines City clinic in the Orlando South market. This is a seat inside a model built for outcomes rather than volume, with smaller patient panels, longer visit times, and a full care team surrounding every physician. You will manage a dedicated panel of Medicare Advantage patients, build lasting relationships with the seniors you serve, and share one scorecard with clinic operators focused entirely on keeping patients healthy. Haines City offers small town warmth with easy access to Central Florida's lakes, parks, and growing communities, making this the right place to build both a practice and a life.
What you will own
- Manage a dedicated panel of senior Medicare Advantage patients with appointment times built for real relationship and clinical depth
- Lead comprehensive primary care visits including annual wellness exams, chronic disease management, and same day acute concerns
- Develop and update individualized care plans that keep patients healthy and out of the hospital
- Partner daily with a care team of nurses, medical assistants, behavioral health, and care coordinators built around your panel
- Review quality and outcomes data regularly and adjust care plans to close gaps and improve patient health
- Order and interpret diagnostic tests, labs, and specialist referrals as part of coordinated, whole person care
- Counsel patients and families on preventive health, medication management, and healthy aging
- Participate in care team huddles and case reviews to align on the highest risk and highest need patients
- Support a culture where physicians and clinic operators share one goal, one scorecard, and one definition of success
What we look for
- Active and unrestricted Florida medical license as an MD or DO
- Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
- Current DEA registration for schedule II through V controlled substances
- Experience or strong interest in senior care and value based, Medicare Advantage focused practice
- Comfort working within a smaller panel model that prioritizes time per patient over volume
- Strong communication skills and a collaborative approach to working with a multidisciplinary care team
- Commitment to preventive care, chronic disease management, and measurable patient outcomes
- Basic Life Support certification, or willingness to obtain within the first 90 days
- Availability for full time, in clinic practice in Haines City
