Primary Care Physician, Fort Pierce

Overview

Team Florida is looking for a full time Primary Care Physician to lead senior focused, value based care at our Fort Pierce clinic on the Treasure Coast. In this model you carry a smaller patient panel and spend real time with every senior, backed by a full care team that handles care coordination, referrals, and outreach so you can focus on medicine. You and your clinic operator share one scorecard built around keeping patients healthy, not how many visits you complete in a day. Fort Pierce is growing fast along the Treasure Coast, and this seat gives a physician the chance to build deep, lasting relationships with the community's seniors from day one of this new organization.

What you will own

• Manage a dedicated panel of senior patients and serve as their primary physician across all aspects of ongoing care
• Conduct longer, unhurried visits that allow time for a full assessment of medical, functional, and social needs
• Build individualized, proactive care plans that emphasize prevention, chronic disease management, and early intervention
• Partner daily with a care team of nurses, medical assistants, care coordinators, and behavioral health support built around your panel
• Review and close quality and preventive care gaps using panel level data and reporting
• Coordinate closely with specialists, hospitalists, and skilled nursing teams when patients need care outside the clinic
• Engage patients and families in shared decision making and health education to support long term wellness
• Participate in regular care team huddles and case reviews to align on the highest risk and highest need patients
• Document care thoroughly and accurately in the electronic health record to support continuity across the care team

What we look for

• Doctor of Medicine or Doctor of Osteopathic Medicine degree from an accredited institution
• Active, unrestricted Florida medical license or eligibility to obtain one
• Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active DEA registration or willingness to obtain one upon hire
• Experience caring for senior patients, with prior experience in a value based or Medicare Advantage setting preferred
• Comfort using panel level quality data, including HEDIS or similar measures, to guide proactive care
• Strong communication skills with patients, families, and a multidisciplinary care team
• Availability for a full time, Monday through Friday outpatient schedule
• Commitment to a collaborative, team based approach to primary care
