Primary Care Physician, Fort Pierce

Overview

Team Florida is looking for a Primary Care Physician to lead a senior focused panel at our Fort Pierce clinic on the Treasure Coast. This is a value based practice built around smaller panels, longer visits, and a full care team, where the entire clinic shares one goal and one scorecard tied to keeping patients healthy rather than counting visits. You will practice medicine the way it should feel, with the time and support to know every patient well and get ahead of their health needs. As Team Florida builds this new company around a well established statewide practice, Fort Pierce offers a physician the chance to help shape a growing coastal community's senior care from the ground up.

What you will own

• Lead a dedicated panel of senior patients with the time and support to deliver proactive, relationship based primary care
• Partner daily with a care team of advanced practice clinicians, nurses, medical assistants, behavioral health, and care coordinators built around your panel
• Develop and manage individualized care plans that emphasize prevention, chronic disease control, and early intervention
• Review outcomes and quality data regularly with clinic leadership to guide how the care team supports each patient
• Coordinate closely with specialists, hospitalists, and skilled nursing partners to keep care connected across every setting
• Engage patients and families in shared decision making, building trust that keeps seniors engaged in their own health
• Document thoroughly in the electronic medical record to support accurate, high quality care coordination
• Mentor and collaborate with advanced practice clinicians and clinical staff to strengthen the team based care model
• Participate in clinic huddles and quality initiatives that keep the whole team aligned around patient outcomes

What we look for

• Active and unrestricted Florida medical license as an MD or DO
• Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active DEA registration or willingness to obtain one before start
• Experience or strong interest in outpatient primary care for senior patients
• Comfort practicing in a value based, team based care model focused on quality and outcomes rather than visit volume
• Working knowledge of Medicare guidelines and Medicare Advantage populations preferred
• Strong communication skills and a collaborative approach to working with a multidisciplinary care team
• Willingness to complete required health screenings, including TB screening, for this patient facing role
• Availability for full time practice at the Fort Pierce clinic
