Primary Care Physician, Bayshore Gardens

Physician Primary Care Bradenton, FL Full time

Overview

Team Florida is seeking a full time Primary Care Physician to lead a senior focused patient panel at our Bayshore Gardens clinic in the greater Tampa Bay region. This role lives entirely inside our value based model, with a smaller panel size, longer visits, and a full care team built around you so that patient health, not visit volume, drives every decision. As Team Florida forms a new company around a long established statewide senior care practice, this is a chance to help shape the physician culture of a growing organization from day one. You will build lasting relationships with seniors across Bayshore Gardens and the surrounding Bradenton and Sarasota communities while practicing medicine the way it was meant to be practiced.

What you will own

  • Manage a dedicated panel of senior patients kept intentionally smaller than fee for service norms so you have real time for every relationship
  • Conduct comprehensive primary care visits that run longer than a typical appointment, addressing acute, chronic, and preventive needs together
  • Partner daily with an onsite care team of nurses, medical assistants, behavioral health staff, and care coordinators to close gaps in care
  • Build and own individualized care plans for patients managing multiple chronic conditions common among seniors, including diabetes, hypertension, and heart failure
  • Track quality and outcomes measures for your panel and work with clinic leadership toward one shared scorecard for physicians and operators
  • Coordinate care transitions with specialists, hospitals, and skilled nursing facilities to keep patients healthy and out of the hospital
  • Lead or join regular care team huddles to review high risk patients and proactively close gaps before they become emergencies
  • Engage patients and families in shared decision making that keeps seniors active partners in their own health
  • Document thoroughly and promptly in the electronic health record to support care coordination and quality reporting

What we look for

  • Active and unrestricted Florida medical license as an MD or DO
  • Board certification or board eligibility in Family Medicine or Internal Medicine
  • Active DEA registration
  • Experience caring for senior or Medicare Advantage patient populations preferred
  • Experience or strong interest in value based, capitated, or risk based primary care
  • Comfort practicing within a collaborative, team based care delivery model
  • Strong communication skills with patients, families, and multidisciplinary care team members
  • Proficiency with electronic health record systems
  • Availability for a full time schedule
