Primary Care Physician, Apollo Beach

Team Florida is hiring a full time Primary Care Physician to lead a senior focused patient panel at our Apollo Beach clinic in the Tampa Bay market. This is a value based seat from day one, built around a smaller panel, longer visits, and a full care team so you can practice medicine the way you trained to, with time to know your patients and the support to act on what you find. Apollo Beach is a waterfront community on the eastern shore of Tampa Bay known for its boating lifestyle and family friendly feel, with easy access to the energy of greater Tampa. As Team Florida builds this new chapter of a long established statewide practice, this is a rare chance to help shape senior primary care in one of the region's fastest growing communities while sharing one goal and one scorecard with your care team and your operators.

What you will own

• Serve as the primary care physician for an adult patient panel, predominantly Medicare eligible seniors, at the Apollo Beach clinic
• Lead each patient's care plan using visits of 30 to 45 minutes to address chronic conditions, preventive screenings, and social needs in a single appointment
• Partner daily with a dedicated care team including medical assistants, care coordinators, behavioral health, and pharmacy to close care gaps before they become emergencies
• Develop and own proactive care plans for a focused panel, following patients across office visits, hospital stays, and transitions home
• Review population health data with clinic leadership to identify patients who need outreach, intervention, or a change in care plan
• Collaborate with specialists and facilities across the Tampa market to coordinate referrals and keep care connected
• Document clearly and efficiently in the electronic health record to support quality measures and continuity of care
• Mentor and guide advanced practice clinicians and support staff as part of a unified clinical team
• Participate in regular care team huddles and quality reviews focused on patient outcomes rather than visit volume

What we look for

• Active and unrestricted Florida medical license as an MD or DO
• Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active DEA registration or eligibility to obtain one upon hire
• Experience or strong interest in caring for senior patients in a value based or risk bearing care model
• Comfort managing a focused panel with an emphasis on quality outcomes, preventive care, and chronic disease management
• Strong communication skills for collaborating with a multidisciplinary care team and engaging patients and families
• Willingness to use data and quality reporting to guide proactive, whole person care
• Ability to work full time in the Apollo Beach clinic with reliable, predictable scheduling
• Commitment to continuous learning and improvement within a team based, patient centered culture
