Overview

Team Florida is looking for a Per Diem Skilled Nursing Facility Physician to round on senior patients recovering in partner skilled nursing facilities across the Tampa Bay area. This role sits inside a value based model built for seniors, where smaller panels, longer visits, and a full care team around every patient make room for the kind of hands on, preventive medicine that keeps people healthy rather than cycling back to the hospital. You will work alongside hospitalists, primary care physicians, and facility staff who share one goal and one scorecard for every patient's recovery. This is a flexible opportunity for a physician who wants meaningful, well supported per diem work in one of Florida's fastest growing senior care markets.

What you will own

  • Round on a flexible per diem schedule at partner skilled nursing facilities throughout the Tampa Bay area
  • Complete timely admission histories and physicals, progress notes, and discharge summaries for patients in post acute skilled nursing stays
  • Adjust medications and treatment plans in real time alongside facility nursing staff to catch complications early and prevent avoidable hospital readmissions
  • Partner with hospitalists, primary care physicians, and specialists so each patient's plan of care stays aligned from hospital to facility to home
  • Join interdisciplinary rounds with facility nursing, therapy, and social work teams to review each resident's progress and needs
  • Communicate directly and compassionately with patients and families about diagnosis, prognosis, and goals of care
  • Document every visit thoroughly to support quality tracking and the shared outcomes scorecard
  • Identify patients who would benefit from palliative care, home health, or a return to a Team Florida primary care clinic
  • Serve as a steady, trusted clinical presence for patients and facility staff during short notice or as needed coverage

What we look for

  • Active and unrestricted Florida medical license, MD or DO
  • Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine
  • Active DEA registration
  • Experience caring for seniors in skilled nursing, post acute, or long term care settings preferred
  • Comfort practicing inside a value based care model that measures success through patient outcomes rather than visit volume
  • Strong communication skills for coordinating with facility staff, families, and a multidisciplinary care team
  • Reliable transportation and willingness to travel between skilled nursing facilities across the Tampa Bay area
  • Availability for flexible, per diem rounding shifts including short notice coverage
  • Current CPR or BLS certification
