Per Diem Hospitalist, Pinellas County
Overview
Team Florida is looking for a per diem hospitalist to round on our senior patients admitted to hospitals across Pinellas County, from Clearwater and Largo to St. Petersburg and Pinellas Park. This role lives inside our value based model, where the goal is keeping every attributed senior healthy rather than maximizing encounters, so you will carry a manageable daily census, spend real time at the bedside, and coordinate closely with each patient's primary care team back at the clinic. You will step in on the days and shifts that fit your schedule, backed by care managers, clinic physicians, and a shared scorecard built around patient outcomes rather than volume. This is a meaningful opportunity for a hospital medicine physician who wants flexible, well supported inpatient work inside one of Florida's most established senior focused primary care networks.
What you will own
- Round daily on Team Florida senior patients admitted to partner hospitals throughout Pinellas County
- Perform admission histories, physical exams, and ongoing daily assessments for hospitalized patients
- Develop and adjust inpatient treatment plans in partnership with hospital based specialists and nursing staff
- Communicate directly with each patient's primary care physician to keep the outpatient care team informed throughout the stay
- Lead discharge planning that sets patients up for a safe transition home or to a skilled nursing facility
- Document clearly and promptly in the electronic medical record to support continuity of care and quality reporting
- Participate in care coordination huddles with case managers and social workers to address discharge barriers early
- Respond to changes in patient status and manage acute issues in collaboration with the appropriate specialists
- Support the clinic care team's outcomes focused goals by helping prevent avoidable readmissions
What we look for
- Active and unrestricted Florida medical license as an MD or DO
- Board certification or board eligibility in Internal Medicine, Family Medicine, or a related primary care specialty
- Active DEA registration
- Prior experience providing inpatient hospital medicine care, ideally with a senior or geriatric patient population
- Comfort practicing within a value based, outcomes driven care model rather than a volume based structure
- Strong communication skills for coordinating with primary care physicians, specialists, and hospital staff
- Current BLS certification, with ACLS preferred
- Ability to work independently and manage a daily inpatient census with sound clinical judgment
- Flexibility to accept per diem shifts across hospitals in the Pinellas County market
