Overview

Team Florida is looking for a per diem Hospitalist to care for our senior patients during the hospital stays that matter most. This is inpatient medicine built on a value based model, where your only job is to help each patient heal and get home safely, not to move through a fixed volume of encounters. You will work alongside our outpatient primary care teams across Florida, stepping in with flexible, as needed shifts at the partner hospitals where our patients are admitted. As we stand up this new company around an established statewide senior care practice, this seat gives an experienced hospitalist the chance to shape how inpatient and primary care work together for seniors across the state.

What you will own

  • Round daily on Team Florida patients admitted to partner hospitals within your assigned region
  • Keep panel size and daily census purposefully manageable so every patient gets a thorough exam and enough time for sound decision making
  • Communicate directly with each patient's outpatient primary care physician and care team throughout the admission
  • Develop and adjust treatment plans based on clinical judgment and the whole picture of each senior's health, not a fixed set of billable steps
  • Partner with hospital case managers and discharge planners to plan a safe and timely transition to home, skilled nursing, or rehabilitation
  • Document a clear discharge summary and personally hand off each patient's status and follow up needs to their primary care team
  • Communicate with patients and families in plain language about diagnosis, treatment, and what recovery will look like
  • Support quality outcomes such as avoiding unnecessary readmissions and closing gaps in care identified during the stay
  • Accept per diem shifts on a flexible basis that fit hospital coverage needs across Florida markets

What we look for

  • Doctor of Medicine or Doctor of Osteopathic Medicine degree from an accredited program
  • Active and unrestricted Florida medical license
  • Board certification or board eligibility in internal medicine, family medicine, or a related specialty
  • Active DEA registration
  • Current hospital privileges or the ability to obtain privileges at Team Florida partner hospitals
  • Experience caring for older adults, ideally within a Medicare Advantage or value based population
  • Comfortable working a per diem schedule with flexible, as needed shifts
  • Strong communication skills for coordinating closely with outpatient primary care physicians and care teams
  • Current BLS certification, with ACLS as required by the facility
