Hospitalist, Pinellas County

Physician Primary Care Pinellas County, FL Per diem

Overview

Team Florida is seeking a per diem Hospitalist to round on our senior patients admitted to hospitals across Pinellas County. In this role you extend our value based primary care model into the inpatient setting, working from one shared scorecard with our clinic based physicians so that every hospitalization is shorter, safer, and better coordinated. You will carry a focused daily census rather than a high volume rotation, giving you the time to manage each patient thoroughly and communicate closely with the outpatient care team. This is a flexible opportunity for a physician who wants meaningful inpatient work on their own schedule while supporting a statewide senior care organization built around keeping patients well.

What you will own

  • Round daily on Team Florida patients admitted to partner hospitals throughout Pinellas County
  • Complete thorough admission histories, physical examinations, and documentation for each patient
  • Diagnose and manage acute and chronic conditions during the inpatient stay with attention to the whole person
  • Communicate directly with each patient's primary care physician and care team to maintain continuity before, during, and after the hospitalization
  • Coordinate timely, safe discharges and transitions to skilled nursing or home based care when appropriate
  • Partner with hospital case managers, specialists, and nursing staff to align on a single plan of care
  • Identify and help close care gaps uncovered during the inpatient stay so outpatient follow up starts immediately
  • Order and interpret diagnostic studies and therapies appropriate to the acuity of each patient
  • Document clearly and promptly in the electronic medical record to support quality tracking and shared outcomes

What we look for

  • Active and unrestricted Florida medical license
  • MD or DO degree from an accredited institution
  • Completion of a residency in Internal Medicine or Family Medicine
  • Board certification or board eligibility in Internal Medicine or Family Medicine
  • Active DEA registration
  • Experience caring for senior patients in an inpatient or hospitalist setting preferred
  • Comfort working within a value based, team based care model focused on outcomes rather than volume
  • Strong communication skills for coordinating with primary care physicians and multidisciplinary hospital teams
  • Availability for flexible per diem shifts covering hospitals across Pinellas County
