Advanced Practice Clinician, Valrico

Overview

Team Florida is looking for a full time Advanced Practice Clinician to join our Tampa-Valrico primary care clinic and help lead senior focused, value based care for the Valrico community. This is a role built around a smaller patient panel, longer visits, and a full care team around you, so your time goes into relationship building and proactive prevention rather than rushing through a schedule. You will work alongside a collaborating physician and a team of care coordinators, medical assistants, and behavioral health support, all sharing one goal and one scorecard focused on keeping patients healthy. What you will own

• Manage a dedicated panel of senior patients with extended visit times that allow for thorough assessment and relationship building
• Conduct annual wellness visits, chronic condition management, and preventive screenings aimed at keeping patients healthy and out of the hospital
• Develop and adjust individualized care plans in partnership with a collaborating physician and the broader clinical team
• Coordinate closely with care coordinators, medical assistants, behavioral health specialists, and pharmacists to address the full picture of each patient's health
• Document thoroughly and accurately in the electronic medical record to support quality measures and continuity of care
• Identify and address social determinants of health that affect patient outcomes, connecting patients to community resources when needed
• Participate in regular care team huddles and quality reviews to track patient outcomes against shared clinic goals
• Engage patients and families in shared decision making to support long term wellness and independence
• Support same day access and timely follow up for patients managing chronic conditions or recovering from a health event

What we look for

• Active and unrestricted Florida license as an Advanced Registered Nurse Practitioner or Physician Assistant
• National certification in a relevant specialty such as Family Medicine, Adult Gerontology, or Internal Medicine
• Active DEA registration or eligibility to obtain one prior to start
• Collaborative practice agreement with a supervising physician in place or ready to be established
• Experience providing primary care to adult or senior patients, with geriatric or value based care experience preferred
• Comfort working within an electronic medical record system for documentation and care coordination
• Current BLS certification
• Strong communication skills and a genuine interest in building long term relationships with patients and families
• Ability to work full time in the Valrico clinic with reliable, in person availability
