Advanced Practice Clinician, Tampa

Overview

Team Florida is looking for an Advanced Practice Clinician to join our Northeast Tampa clinic and help lead senior primary care for a smaller, focused patient panel. This is a value based role, which means longer visits, proactive care planning, and a full team built around every patient so you can practice at the top of your license instead of racing a schedule. You will manage a longitudinal panel of Medicare age patients in the growing Northeast Tampa community, backed by physicians, care coordinators, and specialists who share your goal and your scorecard. This seat matters now because Tampa's senior population is expanding quickly and our model depends on clinicians who want to keep patients well, not just treat them when they are sick.

What you will own

• Serve as a primary care provider for an assigned panel of senior patients, managing their ongoing health in partnership with a supervising or collaborating physician
• Conduct comprehensive visits that give real time for history taking, medication review, and preventive screening rather than rushed encounters
• Develop and adjust individualized care plans for chronic conditions common in a senior population, such as diabetes, hypertension, and heart disease
• Partner daily with a care team of nurses, medical assistants, behavioral health, and care coordinators to close gaps in care
• Document and track quality measures and health outcomes that reflect the value based scorecard shared across the clinic
• Coordinate transitions of care with hospitals, skilled nursing facilities, and specialists to keep patients supported outside the clinic walls
• Engage patients and families in shared decision making, education, and goal setting to support long term wellness
• Participate in clinic huddles and case reviews that keep the whole care team aligned on each patient
• Support same day access and proactive outreach so at risk patients are seen before small issues become emergencies

What we look for

• Active Florida license as a Nurse Practitioner or Physician Assistant, in good standing
• Board certification in a relevant primary care specialty, such as Family Medicine, Adult Gerontology, or Internal Medicine
• Active DEA registration or eligibility to obtain one
• Master's or doctoral degree from an accredited Nurse Practitioner or Physician Assistant program
• Experience caring for senior or Medicare age patients strongly preferred
• Comfort practicing in a value based, team based care model with smaller panels and outcome focused goals
• Strong communication skills for collaborating with physicians, care coordinators, and specialists
• Willingness to work full time in the Northeast Tampa clinic with a consistent, patient centered schedule
• Bilingual ability in Spanish or another language relevant to the Tampa community is a plus
