Advanced Practice Clinician, Tampa
Overview
Team Florida is looking for an Advanced Practice Clinician to manage a dedicated panel of senior patients at one of our Tampa Bay area clinics. This role sits inside a true value based model, where your panel is sized for real relationships, your visits run longer than typical primary care appointments, and you are surrounded by a full care team built to support proactive, preventive care rather than rushed volume. What you will own
- Manage a dedicated panel of senior primary care patients as their trusted advanced practice clinician, with longer visit times built into the schedule
- Conduct comprehensive visits that include chronic disease management, preventive screenings, and proactive health assessments tailored to older adults
- Partner daily with a supervising or collaborating physician and a full clinic care team including medical assistants, care coordinators, and pharmacy and behavioral health support
- Develop and adjust individualized care plans that keep patients healthy at home and reduce avoidable hospital and emergency visits
- Use patient data and quality dashboards to close gaps in care, track outcomes, and guide same day clinical decisions
- Coordinate care across specialists, hospitals, and skilled nursing facilities so each patient experience stays connected and consistent
- Educate patients and families on chronic condition management, medications, and preventive health steps in plain, supportive language
- Document thoroughly and accurately in the electronic medical record to support continuity of care and quality reporting
- Participate in clinic quality initiatives and team huddles focused on shared outcomes for the senior patient panel
What we look for
- Active and unrestricted Florida license as an Advanced Practice Registered Nurse or Physician Assistant
- Board certification through ANCC, AANP, or NCCPA, or the applicable certifying body for your license type
- Active DEA registration or the ability to obtain one prior to start
- Master's or doctoral degree from an accredited nurse practitioner or physician assistant program
- Experience providing primary care to adult or senior patients, with value based or Medicare Advantage experience preferred
- Comfort practicing within a collaborative or supervising physician agreement as required under Florida law
- Strong skills in chronic disease management, preventive care, and care coordination for older adults
- Working knowledge of electronic medical records and comfort using clinical data to guide patient care
- Availability for a full time schedule based at a Tampa Bay area clinic
