Advanced Practice Clinician, Bayway
Overview
Team Florida is looking for an Advanced Practice Clinician to join our senior primary care team at our Bayway clinic in Tampa. This role practices inside a true value based model, carrying a smaller patient panel, spending real time with each senior, and working alongside a full care team united around one goal and one scorecard, the health of every patient on the panel. You will manage chronic conditions, lead preventive visits, and help patients stay well between appointments rather than simply treating them when something goes wrong. As Team Florida builds this new company on an established statewide practice, this is a chance to shape senior care in the Bayway community for years to come.
What you will own
- Manage a focused panel of senior patients with the time and support needed to know each one well
- Conduct comprehensive primary care visits including annual wellness exams, chronic disease management, and preventive screenings
- Develop and adjust individualized care plans in partnership with the physician led care team
- Coordinate closely with care managers, medical assistants, behavioral health specialists, and social workers to address the full picture of each patient's health
- Monitor and manage chronic conditions such as diabetes, hypertension, and heart disease to prevent avoidable hospitalizations
- Document care thoroughly and accurately in the electronic health record to support quality tracking and outcomes reporting
- Educate patients and families on disease prevention, medication management, and healthy aging
- Participate in daily huddles and care team meetings focused on shared quality goals for the panel
- Build lasting relationships with patients and their families that support trust and long term engagement in their own health
What we look for
- Active Florida license as a Nurse Practitioner or Physician Assistant
- Board certification through ANCC, AANP, or NCCPA as applicable to license type
- Active DEA registration or eligibility to obtain one
- Master's degree from an accredited Nurse Practitioner or Physician Assistant program
- Experience providing primary care to adult or senior patients preferred
- Comfort working within a value based, team based care model focused on outcomes rather than visit volume
- Strong communication skills and genuine interest in building relationships with senior patients and their families
- Ability to collaborate effectively with physicians and a multidisciplinary care team
- Availability for full time, Monday through Friday clinic hours at the Bayway location
