Overview

Team Florida is looking for a full time Advanced Practice Clinician to join our senior focused primary care clinic in Crystal Grove, part of the greater Tampa market. In this value based role you will carry a smaller, dedicated patient panel and spend real time with every senior you see, backed by a full care team that shares your goal of keeping patients healthy rather than simply counting visits. You will work side by side with a primary care physician, care coordinators, and specialty partners so that every plan of care is proactive, coordinated, and built around the whole person. This is a chance to help stand up a new chapter for an established Tampa practice, with the stability of a statewide organization and the close knit feel of a single neighborhood clinic.

What you will own

  • Manage a smaller panel of senior primary care patients with the time needed for thorough, relationship based visits
  • Partner daily with your supervising or collaborating physician and the full clinic care team on shared patient goals
  • Complete comprehensive visits including annual wellness exams, chronic disease management, and preventive screenings
  • Develop and adjust individualized care plans that reflect each patient's medical, functional, and social needs
  • Order, review, and act on diagnostic testing and coordinate timely follow up with specialists
  • Prescribe and manage medications within your scope, monitoring for safety and effectiveness over time
  • Document thoroughly in the electronic medical record to support continuity of care and quality reporting
  • Engage patients and families in education that supports self management between visits
  • Participate in team huddles and quality reviews that track outcomes across the shared clinic scorecard

What we look for

  • Active, unrestricted Florida license as an Advanced Registered Nurse Practitioner or Physician Assistant
  • Board certification through AANP, ANCC, or NCCPA appropriate to your license type
  • Valid DEA registration or the ability to obtain one prior to start
  • Experience providing primary care to adult or senior patients, ideally within a Medicare Advantage or value based population
  • Comfort managing patients with multiple chronic conditions and complex needs
  • Strong documentation skills and comfort working within an electronic medical record
  • Ability to collaborate closely with physicians and a multidisciplinary care team on shared quality goals
  • Current CPR or BLS certification
  • Availability for full time, weekday clinic hours in the Crystal Grove and greater Tampa market
