Advanced Practice Clinician, South Florida
Overview
Team Florida is looking for a PRN Advanced Practice Clinician to provide senior focused primary care across our South Florida clinics. This is a value based seat, which means smaller patient panels, more time in every visit, and a full care team built around each patient so you can practice proactive, preventive medicine instead of chasing volume. You will step into clinics across the South Florida market on a flexible, as needed schedule, delivering the same high touch care our full time clinicians provide every day. What you will own
- Conduct comprehensive wellness visits and annual assessments for a panel of senior patients across assigned South Florida clinics
- Manage chronic conditions common to aging patients, including diabetes, hypertension, and heart disease, with a focus on preventing complications before they happen
- Order, review, and act on diagnostic tests and labs, adjusting care plans to keep each patient on track toward their health goals
- Work side by side with physicians, care coordinators, behavioral health specialists, and support staff as one care team with one shared goal for every patient
- Document thorough, accurate notes in the electronic health record to support continuity of care across visits and clinics
- Educate patients and families on chronic disease management, medication adherence, and healthy aging in plain, encouraging language
- Participate in care coordination huddles and quality reviews to close gaps in care and support strong outcomes across the panel
- Provide reliable, professional coverage at different clinic locations within the South Florida market as PRN scheduling needs arise
- Support quality and outcome measures that reflect the value based model, working toward the same scorecard as clinic leadership
What we look for
- Active Florida license as an Advanced Registered Nurse Practitioner or Physician Assistant
- National board certification appropriate to licensure, such as ANCC, AANP, or NCCPA
- Active DEA registration or eligibility to obtain one
- Current BLS certification
- Experience providing primary care to adult or senior patients, with value based or geriatric care experience preferred
- Comfort working independently within a collaborative, physician supported care team model
- Reliable transportation and willingness to work across multiple clinic locations within the South Florida market
- Strong documentation skills and comfort working within an electronic health record system
- Availability and flexibility to work PRN shifts based on clinic coverage needs
