Advanced Practice Clinician, Tampa

Overview

Team Florida is seeking a full time Advanced Practice Clinician to join our senior primary care team in the Tampa North market. In this value based care seat you will carry a deliberately smaller patient panel, spend real time with every senior you see, and lean on a full clinic care team so your focus stays on prevention and outcomes rather than volume. You will practice in the heart of Tampa, building lasting relationships with patients who return to the same clinic and the same team visit after visit. This is a chance to help shape a brand new company built on an established statewide practice, right as it takes its next step forward in Florida senior care.

What you will own

• See a defined panel of senior patients for longitudinal primary care, with visit lengths built for real conversation and thorough assessment
• Develop and manage individualized care plans for chronic disease, preventive screening, and wellness goals in partnership with the clinic physician and care team
• Round daily with medical assistants, care coordinators, and behavioral health staff to close gaps in care before they become complications
• Order and interpret diagnostics, manage medications, and adjust treatment plans based on each patient's evolving needs
• Document thoroughly in the electronic health record to support quality measures and whole person care coordination
• Partner with specialists, hospitalists, and skilled nursing teams to keep transitions of care smooth for every patient
• Engage patients and families in shared decision making around goals of care, screenings, and lifestyle changes
• Track panel level quality and outcome measures alongside clinic leadership as part of one shared scorecard
• Participate in daily huddles and ongoing clinical education to keep the whole team aligned on patient outcomes

What we look for

• Active Florida license as an Advanced Registered Nurse Practitioner or Physician Assistant
• National board certification appropriate to licensure, such as ANCC, AANP, or NCCPA
• Active DEA registration or ability to obtain one prior to start
• Current BLS certification
• Experience caring for adult and geriatric patients in a primary care or internal medicine setting
• Comfort managing a full panel of patients with chronic conditions common in a senior population
• Collaborative mindset and strong communication skills for working within a multidisciplinary care team
• Willingness to practice within a value based, outcomes focused care model
• Ability to work full time on-site in the Tampa North market
