Advanced Practice Clinician, Pinellas
Overview
Team Florida is seeking a full time Advanced Practice Clinician to manage a panel of senior patients at our Pinellas primary care clinic in the Tampa North market. This is a value based seat, which means a smaller panel, more time in every visit, and a care team built around you rather than a schedule built around volume. You will practice alongside a collaborating physician and a full support staff whose only job is to help your patients stay healthy, with quality and outcomes as the shared measure of success for you and clinic leadership alike. What you will own
- Manage a defined panel of senior primary care patients with longer visit times built into the schedule
- Deliver longitudinal, relationship based care including annual wellness visits, chronic disease management, and same day acute concerns
- Partner with a collaborating physician and an in clinic care team of nurses, medical assistants, and care coordinators on every patient plan
- Close care gaps proactively and drive preventive screenings that keep the panel healthy rather than reacting to illness
- Document and coordinate care across specialists, hospitals, and skilled nursing facilities to keep the patient at the center
- Review daily huddles and panel quality data with the clinic team to identify patients who need outreach or a closer look
- Educate patients and families on chronic condition management, medication adherence, and social factors affecting health
- Participate in clinic quality initiatives tied to the same outcomes scorecard shared by clinicians and clinic leadership
- Support a culture of trust and continuity so patients see a familiar, dedicated care team at every visit
What we look for
- Active Florida license as a Nurse Practitioner or Physician Assistant in good standing
- National board certification appropriate to license, such as ANCC or AANP for Nurse Practitioners or NCCPA for Physician Assistants
- Active DEA registration or eligibility to obtain one
- Experience or strong interest in senior focused primary care and chronic condition management
- Comfort practicing in a value based, team based care model with a defined patient panel
- Strong communication skills for working closely with a collaborating physician and a multidisciplinary clinic team
- Experience with electronic health record systems and accurate clinical documentation
- Ability to build lasting relationships with patients and families across longer, unhurried visits
- Availability for a full time schedule based in Pinellas within the Tampa North market
