Overview

Team Florida is looking for an Advanced Practice Clinician to join our West Sanford clinic and manage a focused panel of senior patients under a true value based model. In this role you carry meaningfully fewer patients than a fee for service clinic so every visit runs long enough for real conversation, a full physical exam, and proactive planning rather than a rushed checklist. You practice alongside a physician led care team that includes a care coach nurse, medical assistant, behavioral health support, and referral coordination, all working from the same quality scorecard you do. West Sanford is one of the fastest growing corners of Seminole County, and this seat lets you build lasting relationships with the seniors who call this historic riverfront community home.

What you will own

  • Serve as the primary clinician for an assigned senior patient panel, delivering wellness visits, annual physicals, and chronic condition management with extended visit times
  • Develop and update individualized care plans that emphasize prevention, early intervention, and closing gaps in care before they become emergencies
  • Diagnose and treat acute and chronic conditions within scope of practice, prescribing and adjusting medications as needed
  • Partner daily with a dedicated care coach nurse, medical assistant, and behavioral health team to coordinate the full needs of each patient
  • Document every visit with the specificity needed to reflect the true complexity of each patient, working alongside quality based coding support
  • Coordinate specialist referrals, hospital transitions, skilled nursing follow up, home health, and durable medical equipment so patients never fall through the cracks
  • Review outcomes and quality metrics regularly with the clinic's medical director to identify opportunities to improve patient health
  • Participate in the clinic's on call rotation to give patients continuity of care outside normal hours
  • Build trust with patients and families through consistent, unhurried, relationship based care

What we look for

  • Active, unrestricted Florida license as a Nurse Practitioner or Physician Assistant
  • Board certification in a primary care focused specialty such as Family Medicine, Adult Gerontology, or a related area recognized by the Florida board
  • Active DEA registration or eligibility to obtain one prior to start
  • Experience caring for senior patients, with prior exposure to value based or capitated care models strongly preferred
  • Comfort managing multiple chronic conditions common in a senior population, such as diabetes, hypertension, and heart failure
  • Strong documentation habits that support accurate, complete reflection of patient complexity
  • Ability to work collaboratively within a team based care model rather than in isolation
  • Current CPR or BLS certification
  • Willingness to participate in an on call rotation supporting the West Sanford clinic panel
