Advanced Practice Clinician, Sanford

Overview

Team Florida is looking for an Advanced Practice Clinician to join our Orlando West Sanford clinic as we build a new, physician led primary care company around a practice that has already earned the trust of seniors across Sanford. In this role you will manage a smaller, purposefully sized patient panel so every visit has the time it deserves, working inside a value based model where your goal and your physician partner's goal are the same: healthier patients and stronger outcomes, not volume. You will be supported by a full clinical team built around you, including medical assistants, care coordinators, pharmacy, and behavioral health, so you can practice at the top of your license. This is a ground floor opportunity to shape senior care in Sanford as our company stands up its own identity on a foundation of established, high performing clinics.

What you will own

• Manage a defined panel of senior patients, delivering longer, unhurried visits focused on prevention and whole person health
• Partner closely with the supervising or collaborating physician to design and adjust individualized care plans
• Conduct annual wellness visits, chronic condition management, and acute visits for adult and senior patients
• Use population health data and care gap reports to proactively reach out to patients before small issues become big ones
• Collaborate daily with medical assistants, RN care coordinators, pharmacists, and behavioral health specialists as one care team
• Order, interpret, and act on diagnostic studies and coordinate specialist referrals within the clinic's care network
• Prescribe and manage medications, including chronic disease regimens common in a senior population
• Document care thoroughly and accurately to support quality measures and the clinic's shared outcomes scorecard
• Participate in team huddles, care team meetings, and clinic quality initiatives aimed at keeping patients well

What we look for

• Active, unrestricted Florida license as an Advanced Registered Nurse Practitioner or Physician Assistant
• National board certification appropriate to licensure, such as ANCC, AANP, or NCCPA
• Master's degree or doctorate from an accredited Nurse Practitioner or Physician Assistant program
• Active DEA registration or ability to obtain prior to start
• Experience in primary care, family medicine, internal medicine, or geriatrics preferred
• Comfort working in a value based, team based care model focused on quality and outcomes
• Strong clinical judgment and comfort managing chronic conditions common among senior patients
• Current BLS certification
• Bilingual English and Spanish skills a plus for serving the Sanford community
