Advanced Practice Clinician, Haines City
Overview
Team Florida is seeking a full time Advanced Practice Clinician to join our senior primary care clinic in Haines City, part of the Orlando South market. This role practices inside a true value based model, carrying a smaller, dedicated patient panel with real time to spend on prevention, chronic condition management, and relationship building rather than rushed visits. You will work alongside a supervising physician and a full care team built around every patient, sharing one scorecard focused on keeping seniors healthy and out of the hospital. What you will own
- Manage a dedicated panel of senior patients, building continuity and trust over repeat visits
- Conduct annual wellness visits, chronic disease management appointments, and same day acute care within extended visit times
- Collaborate daily with a supervising physician and an interdisciplinary team including RNs, medical assistants, care coordinators, pharmacists, and behavioral health specialists
- Order, interpret, and act on diagnostic testing and labs to guide proactive treatment plans
- Develop and update individualized care plans that reflect each patient's goals, risks, and social needs
- Document encounters accurately and promptly in the electronic medical record to support coordinated, compliant care
- Review panel level quality and outcomes data, including preventive screening and chronic condition control measures, and adjust outreach accordingly
- Educate patients and family caregivers on self management, medication adherence, and preventive health steps
- Participate in regular care team huddles and case reviews to close gaps in care before they become emergencies
What we look for
- Active, unrestricted Florida license as an Advanced Registered Nurse Practitioner or Physician Assistant
- National board certification appropriate to license type, such as ANCC, AANP, or NCCPA
- Active DEA registration or eligibility to obtain one prior to start
- Current BLS certification, with ACLS preferred
- Experience providing primary care to adult or senior patients, ideally within a value based or managed care setting
- Comfort using electronic medical records and coordinating care across a multidisciplinary team
- Strong communication skills for building trust with patients, families, and caregivers
- For Nurse Practitioners, an active or eligible collaborative practice agreement in accordance with Florida law
- Availability for full time clinic based hours in Haines City
