Advanced Practice Clinician, Haines City

Overview

Team Florida is seeking an Advanced Practice Clinician to join our senior focused primary care clinic in Haines City, part of the Orlando South market. This role operates inside a true value based model, carrying a smaller patient panel, booking longer visits, and working alongside a full care team so that quality and outcomes, not volume, define success. You will share one goal and one scorecard with your collaborating physician and the clinic operator, all pointed at keeping Haines City seniors healthy and out of the hospital. This is a seat for a clinician who wants to practice proactive, relationship driven medicine and help shape a growing statewide organization from its earliest days in this community.

What you will own

• Manage a dedicated panel of senior patients, delivering comprehensive primary care including annual wellness visits, chronic disease management, and same day acute concerns
• Spend extended time with each patient to build trust, understand their full health picture, and close care gaps before they become emergencies
• Collaborate daily with a supervising physician, nurse practitioners or physician assistants, medical assistants, and care coordinators as one unified care team
• Develop and adjust individualized care plans focused on preventive screenings, medication management, and avoidable hospitalization reduction
• Order, review, and act on diagnostic tests and labs, coordinating referrals to specialists while keeping care centered in the clinic
• Document thoroughly in the electronic health record and communicate clearly with patients through visits, phone calls, and the patient portal
• Participate in regular care team huddles, quality reviews, and outcomes discussions tied to the clinic's shared scorecard
• Support patients and families with education on chronic conditions, healthy aging, and navigating the Medicare Advantage benefit
• Engage with the Haines City community to build the clinic's reputation as a trusted home for senior primary care

What we look for

• Active Florida license as a Nurse Practitioner or Physician Assistant in good standing
• National board certification through ANCC, AANP, or NCCPA as applicable to your credential
• Active DEA registration or willingness to obtain one prior to start
• Current BLS certification
• Experience providing primary care to adult or senior patients, with Medicare or Medicare Advantage experience strongly preferred
• Comfort practicing within a value based, team based model that measures success through patient outcomes and quality metrics rather than visit volume
• Strong communication skills and genuine comfort building long term relationships with senior patients and their families
• Proficiency with electronic health record systems and telehealth tools
• Ability to work full time on site in Haines City as part of the Orlando South market
