Advanced Practice Clinician, Fort Pierce

Overview

Team Florida is looking for an Advanced Practice Clinician to join our senior focused primary care clinic in Fort Pierce, part of our growing Treasure Coast market. This role practices inside a true value based care model, carrying a smaller patient panel, spending real time with each senior, and working alongside a physician led care team that shares one goal and one scorecard for keeping patients healthy. You will build lasting relationships with the same patients visit after visit, catching problems early instead of chasing them later. This is a chance to help shape a new chapter of senior care in Fort Pierce, where your clinical judgment and your patients' outcomes drive everything you do.

What you will own

• Manage a dedicated panel of senior patients, delivering comprehensive primary care across annual wellness visits, chronic disease management, and acute concerns
• Conduct longer, unhurried visits designed to fully assess each patient's physical, cognitive, and social needs
• Partner daily with a supervising physician and an integrated care team of nurses, medical assistants, care coordinators, and behavioral health support
• Build individualized, proactive care plans that close gaps in care before they become emergencies
• Order, review, and act on diagnostic testing, labs, and imaging to guide ongoing treatment
• Prescribe and manage medications within Florida scope of practice, adjusting plans as patient conditions change
• Document thoroughly in the electronic health record to support continuity of care and quality reporting
• Engage patients and families in shared decision making around chronic conditions, preventive screenings, and healthy aging
• Participate in regular care team huddles and quality reviews focused on patient outcomes rather than visit counts

What we look for

• Active Florida license as a Nurse Practitioner or Physician Assistant in good standing
• Board certification through a recognized national certifying body appropriate to your credential
• Active DEA registration or eligibility to obtain one prior to start
• Graduate degree from an accredited Nurse Practitioner or Physician Assistant program
• Experience providing primary care to adult or geriatric patients, ideally within a value based or senior focused setting
• Comfort managing multiple chronic conditions common among older adults, such as diabetes, hypertension, and heart disease
• Strong communication skills for collaborating with physicians, care team members, and patients' families
• Willingness to work full time in the Fort Pierce clinic and build a consistent presence with your patient panel
• Current CPR or BLS certification
