Advanced Practice Clinician, Fort Pierce

Overview

Team Florida is seeking a full time Advanced Practice Clinician to deliver senior focused primary care at our Fort Pierce clinic on the Treasure Coast. This role lives inside our value based model, where you carry a smaller patient panel, spend real time with every senior, and lean on a full care team built around you, with physicians, operators, and clinicians all working from one goal and one scorecard focused on quality and outcomes. You will practice at the top of your license, building lasting relationships with patients who are managing chronic conditions and working proactively to stay healthy. As Team Florida grows its presence across Fort Pierce and the wider Treasure Coast, this is a seat for a clinician who wants to practice medicine the way it was meant to be practiced, with the time and support to do it right.

What you will own

• Manage a focused panel of senior patients, delivering comprehensive primary care including annual wellness visits, chronic disease management, and preventive screenings
• Partner closely with a collaborating physician and a full care team of medical assistants, care coordinators, and clinical pharmacists to keep every patient on track
• Spend extended time with each patient to build trust, uncover health risks early, and create individualized care plans
• Order, perform, and interpret diagnostic tests and develop treatment plans grounded in evidence based medicine
• Document thoroughly and accurately in the electronic health record to support continuity of care and quality reporting
• Coordinate referrals and transitions of care with specialists, hospitals, and community resources to keep patients supported outside the clinic
• Engage patients and families in shared decision making around preventive care, medication management, and wellness goals
• Participate in regular care team huddles and quality reviews focused on patient outcomes rather than visit counts
• Support patient outreach and engagement efforts that keep the Fort Pierce panel connected to proactive, year round care

What we look for

• Active Florida license as an Advanced Registered Nurse Practitioner or Physician Assistant, or eligibility to obtain licensure before start
• National certification through a recognized body such as AANP, ANCC, or NCCPA appropriate to your license type
• Active or obtainable Florida DEA registration
• At least one year of clinical experience as a Nurse Practitioner or Physician Assistant, ideally in primary care or a related setting
• Comfort and confidence caring for a primarily Medicare aged senior population
• Strong interpersonal and communication skills suited to building long term patient relationships
• Ability to work collaboratively within a physician led, team based care model
• Willingness to practice according to value based, outcomes focused principles rather than high volume visit schedules
• Availability for a full time, Monday through Friday clinic schedule in Fort Pierce
