Advanced Practice Clinician, Apollo Beach
Overview
Team Florida is looking for an Advanced Practice Clinician to join our senior focused primary care team in Apollo Beach. This is a value based role built around smaller patient panels, longer visits, and a full care team that works alongside you so your time goes to the patient in front of you, not paperwork or rushed appointments. You will practice at the top of your license within our Tampa market clinic, partnering closely with a collaborating physician and a care team that shares one goal and one scorecard built on patient outcomes. With growth continuing across our Apollo Beach community, this is a strong time to build a lasting career caring for seniors in one of Tampa Bay's fastest growing areas.
What you will own
- Manage a dedicated panel of senior patients with longer scheduled visit times built for relationship based care
- Perform comprehensive wellness visits, annual assessments, and chronic condition management for adults 65 and older
- Develop and update individualized care plans focused on prevention, early intervention, and closing care gaps
- Collaborate daily with a supervising physician and an interdisciplinary care team including medical assistants, care coordinators, and behavioral health support
- Order and interpret diagnostic studies and coordinate specialist referrals within the clinic's value based network
- Document care thoroughly in the electronic medical record to support accurate, timely, and complete patient histories
- Engage patients and families in shared decision making around ongoing health goals and social needs
- Track quality and outcome measures tied to the clinic's shared scorecard and participate in regular care team reviews
- Build trusted, long term relationships with patients that support continuity of care across the Apollo Beach community
What we look for
- Active, unrestricted Florida license as a Nurse Practitioner or Physician Assistant
- Board certification in good standing through ANCC, AANP, or NCCPA as applicable to licensure

- Current BLS certification
- Availability for full time, Monday through Friday clinic hours in Apollo Beach
