Advanced Practice Clinician, Dr. Phillips

Overview

Team Florida is looking for an Advanced Practice Clinician to join our senior primary care team in the Dr. Phillips neighborhood of Orlando. This is a value based primary care role where your panel stays small by design, your visits run longer, and a full care team works alongside you so your time goes toward the patient in front of you rather than the clock. You will practice in one of the Orlando South market's established clinics, supported by physicians, care coordinators, pharmacy, and behavioral health, all sharing one scorecard built on keeping patients healthy rather than counting encounters. With Team Florida forming as a new statewide organization around decades of senior care experience in this community, this is a seat where an experienced clinician can shape how primary care is delivered to Dr. Phillips area seniors for years to come.

What you will own

• Manage a dedicated panel of senior patients with the time and support to know them well
• Conduct comprehensive visits including annual wellness exams, chronic disease management, and preventive screenings
• Develop and adjust personalized care plans in partnership with the patient's primary care physician
• Order and interpret diagnostic studies and coordinate specialist referrals as part of a proactive care plan
• Partner daily with care coordinators, pharmacists, behavioral health specialists, and medical assistants to close gaps in care
• Document care thoroughly in the electronic medical record to support quality measures and continuity of care
• Identify patients at risk for decline and intervene early to prevent avoidable hospitalizations
• Engage patients and families in shared decision making around health goals and treatment plans
• Participate in team huddles and quality reviews that keep the care team aligned around patient outcomes

What we look for

• Active Florida license as a Nurse Practitioner or Physician Assistant, or the ability to obtain licensure prior to start
• Certification through a recognized national board such as AANP, ANCC, or NCCPA
• Active DEA registration or the ability to obtain one prior to employment
• At least one year of clinical experience as a Nurse Practitioner or Physician Assistant
• Comfort managing a primarily Medicare age senior patient population
• Experience with or strong interest in value based, team based primary care
• Strong communication skills for collaborating with physicians and a multidisciplinary care team
• Bilingual English and Spanish skills a plus
• Availability for a full time schedule based in the Dr. Phillips clinic
