Overview

Team Florida is hiring an Advanced Practice Clinician for our Dr. Phillips clinic, part of the Orlando South market serving seniors across southwest Orange County. This is a value based primary care role built around a smaller patient panel, longer visit times, and a full care team that shares one scorecard for keeping patients healthy. You will manage your own panel of Medicare age patients, building the kind of longitudinal relationships that come from time and continuity rather than volume. With Dr. Phillips growing as a destination for families and retirees alike, this is the right moment to plant roots in a practice built for quality, not quotas.

What you will own

  • Manage a dedicated panel of senior patients, serving as their primary point of contact for ongoing primary care
  • Conduct wellness visits, annual physical exams, and comprehensive health assessments with enough time to address the full picture
  • Diagnose and treat acute and chronic conditions common among seniors, adjusting care plans as health status changes
  • Partner daily with physicians, care coaches, behavioral health specialists, and clinic staff to close gaps in care
  • Develop and update individualized care plans focused on prevention, chronic disease management, and quality outcomes
  • Document thoroughly and accurately in the electronic health record to support care coordination and quality reporting
  • Engage patients and families in shared decision making and health education tailored to their goals
  • Coordinate referrals and transitions of care with specialists, hospitals, and skilled nursing facilities as needed
  • Participate in clinic huddles and quality reviews that track outcomes across the care team

What we look for

  • Active Florida license as a Nurse Practitioner or Physician Assistant in good standing
  • Board certification through an accredited national certifying body appropriate to your license
  • Active DEA registration or eligibility to obtain one prior to start
  • Graduate degree from an accredited Nurse Practitioner or Physician Assistant program
  • Experience providing primary care to adult or senior patients, ideally in a value based or team based setting
  • Comfort working within a collaborative practice model alongside a supervising or collaborating physician as required by Florida law
  • Strong documentation skills and familiarity with electronic health record systems
  • Bilingual ability in Spanish or Creole is a plus given the diversity of the Dr. Phillips patient community
  • Availability for full time clinic hours with the flexibility that senior care sometimes requires
